| Objective: The purpose of this subject is to investigate the relationship amongdifferent structural changes of heart,atrial or ventricular arrhythmias and syndromedifferentiation in essential hypertensive by the method of syndrom differentiation intraditional Chinese medicine combining with the modern medicine. clinical guidance totake timely measures to correct and effective intervention.Methods: According to the standards for the differentiation of syndrome about theclinical research instructive principles in the treatment of hypertension defined in thePrinciple of Clinical Research for New Traditional Herbs set by the Ministry of Health in2005, Will be100cases of hypertensive patients,57males and43females,the patients weredivided into4groups: flaring up of liver fire group, yin deficiency yang hyperactivitygroup,excessive accumulation of dampness phlegm syndrome group and yin yangconcurrent deficiency group.the patients were studied by echocardiography.themeasurements were as follows:LAd,LVDd,IVSd,LVPWd and Their arrhythmias weremonitored by24-houre ambulatory electrocardiogram.Results:1.In terms of gender four TCM syndrome distribution have no obvious Corr-elation,But with age and course,there have obvious Correlation.2.Comparison of structuralchanges of heart:(1) Comparison of Left Atrial Dimension:left Atrial Dimension of Thefour TCM syndrome type has difference (P<0.05).especially The flaring up of liver firegroup and yin yang concurrent deficiency group has significant difference(P<0.01).(2)Co-mparison of indexes of left ventricular structure:with liver-fire hyperactivity type andexcessive phlegm-dampness, there have no significant statistical difference in every indexes of left ventricular structure.Most indexes of left ventricular structure in the flaringup of liver fire group and excessive accumulation of dampness phlegm syndrome groupwere significantly lower than those in the yin yang concurrent deficiency group(P <0.05).The indexes of LVEDd and LVMI in the yin deficiency yang hyperactivity group weresignificantly lower than those in the yin yangconcurrent deficiency group(P<0.05).Theindexes of LVPWd and LVMI in the flaring up of liver fire group were significantly lowerthan those in the yin deficiency yang hyperactivity group(P<0.05). The indexes of LVPWdin excessive accumulation of dampness phlegm syndrome group was significantly lowerthan those in the yin deficiency yang hyperactivity group(P<0.05),3.Comparison of atrialor ventricular arrhythmias:(1)All the enrolled100patients of hypertension,85patientswith arrhythmias,accounts for(85) percent;(2) differentiation syndrome of inessentialhypertensive has the relation with arrhythmias(P<0.05),yin yang concurrent deficiencygroup are much easier to have arrhythmia than other groups;especially the occurrence ofatrial arrhythmia(3) Left atrial enlargement and atrial arrythmias have significantcorrelation(P<0.05),The occurrence of Left ventricular hypetrophy and ventriculararrhythmias have significant correlation (P<0.05).Conclusion:1. The personality characteristics of hypertension,such as age, course, ismarkedly associated with the syndrom of traditional Chinese medicine.2.All the patientswith essential hypertension of different TCM syndromes had left heart remodeling ofdifferent degree in ultrasonic cardiogram, which were more obvious in the hypertensivepatients with yin yang concurrent deficiency.3.The syndrome type of TCM in essentialhypertensive patients have a higher incidence of atrial arrhythmias, especially yin yangconcurrent deficiency group.4.The left atrial expansion will increase the occurrence ofatrial arrhythmia.There is a relationship between arrhythmia in essential hypertension andleft ventricular hypertrophy.5. Combing different Syndrome of traditional Chinesemedicine with the technology of Cardica Doppler ultrasound has an important significanceof theprevention and treatment of hypertension associated with heart damage. |
