Background and objects:Cervical spondylosis is commonly appeared in regular clinical practice of orthopaedics. Under the rapid technological development of imaging diagnosis, the increased detection of cervical spondylosis provides chances for early surgical management of these lesions.Between April2011and January2012, surgical treatment were performed on40patients suffered from cervical spondylosis by different approaches. Case series illustrate the diverse approaches commonly used for cervical spondylosis in surgical treatment are regularly reported. Comparing between two approaches, we tried to analyze the effectiveness and then find out the indication of treatment for a particular case.Methods:1. Collect the clinical data of selected patients suffered from cervical spondylosis2. Follow up of the patients and make detection to find the change of symptoms and the imaging appearance.3. Choose the suitable surgical indications by comparing the data.Result:All the40patients are validated by CT and/or MRI scan after symptomatic diagnosis.26patients underwent posterior surgery, while the other14patients were performed combined anterior-posterior treatment. All the patients’ clinical symptoms were improved in various degrees. Under statistical conclusion, the later clinical outcome of two approaches are considered as the same, while the anterior approach displays more effectiveness in early stage, but also higher complication rate and economic unfriendly.Conclusions:The topic of selection in surgical approaches for treatment of cervical spondylosis is still in controversial. But under this study, several viewpoints can be exhibited as final conclusions.As a progressive degenerative pathologic procedure, cervical spondylosis needs comprehensive diagnosis from history to symptoms followed by imaging materials. Conservative treatments are helpful but can only serve as auxillary means. Act as standardized method for neurologic imaging, MRI scans provide clear view for periphery structures of spinal cord. Comprehensive study of MRI scans with CT and plain films is of great significance in the guidance for surgical strategy. Different approaches display the same later clinical outcome. Simple posterior approach is of lower complication rate and economic friendly.Combined anterior-posterior approach is suitable for particular patients in simple lesions near nerve roots. Non-fusion technologies now are not in perfect satisfaction of physiological structures, but show various advantages to traditional fusion methods.Surgical skills of surgeons should be trained frequently in common practices, which the only way providing benefits for all patients suffered from spinal lesions.
