Background Since laparoscopic techniques were introduced, its small trauma, less bleeding, Painless, shorter hospitalization time, rapidly gastrointestinal function recovery and less postoperative pelvic adhesion, beautiful incision is widely applied in clinic. Laparoscopic techniques which can not only treat benign but also malignancy have achieved remarkable progress.Objective To investigate the feasibility and safety of LAVH for enlarged uteri.Methods40cases performed LAVH on patients with uteri equivalent to12-20weeks gestational size were compared with40cases uteri less than12weeks gestational size. We used comprehensive procedures, including elevation of the puncture sites, inject water to the space between uterine and bladder, diminution of uterus size during the operation, to compare operating time, bleeding loss, complication rate, anus exhaust time after operation, rate of postoperative complication, recovery information of operation site, hospital stays of the two groups.Results No case was conversed to laparotomy in all patients, and there was no significant difference in short and long-term complications between the two groups. But the operation time of study group was significant longer than the control group, and blood loss of study group was significant more than the control group.The study group with operation time2.52±0.69h, the control group with operation time1.92±0.27h (t=5.03, P<0.001), the difference between the two groups was significant. The study group with blood loss81.1±18.3ml, the control group with operation time58.5±14.7ml(t=5.99,P<0.001),the difference between the two groups was significant.The anus exhaust time after operation complication rate and hospital stay of study group was no significant longer than the control group.Conclusion LAVH is a safe and efficient way to manage benign uterine disease. Despite increased operating time and blood loss, LAVH can be safely performed for enlarged uterus.
