Objective To explore the clinical features and the treatment measure of massivecerebral infarction in the First People′s Hospital of ChenZhou.Methods The clinical data of53cases from July2008to January2011wereanalyzed retrospectively. These data include its pathogenesis, clinical features,laboratory examinations, complication, therapy, prognosis,and so on in First People’sHospital of Chenzhou City, the affiliation Hospital of South China.Results1.We summed up the common cause of massive cerebral infarction such ascerebral artriosclerosis, high blood pressure and atrial fibrillation;2.There were33male patients and20female patients in53MBL patients. The cases were middle or oldaged.The mean age was61.82±12.28(33-88years);3. The etiology includedhypertension(41cases), atrial fibrillation(15cases), diabetes mellitus(8cases), heartdisease(29cases), and so on;4.Its clinical features included sudden onset rapiddevelopment, highly mortality, poor outcome. The main symptoms were contralateralhemiplegia hmianopia, disorders of consciousness, gaze palsy and intracranialhypertension.41cases happened in quiescent condition,12cases happened in activecondition,46cases had disturbance of consciousness in various degree at the beginningof a disease. Among them, There were7cases of hypersomnia,5cases of lethargy,19cases of narcosis,11cases of mental confusion,28cases of physical hemiplegia,2casesof convulsion,19cases of eyeball side viewing maladjustment. We found14negativecases by early CT images.9cases in Hemorrhagic cerebral infarction,28cases in Leftcerebral infarction,18cases in Right cerebral infarction. It shows that Massive cerebralinfraction is a acute, severity complication disease. CT or MRI results to showinfarction of different vascular territory,there were significant difference between CTand MRI (p>0.05);5.There were41cases with pneumonia,18cases with stress ulcer,6cases with cardiac arrhythmia in53MBL patients, and so on;6.Among all cases,31cases survived,8were discharged automatically for aggravation their conditions and4 cases died, there were no significant difference between the two medical treatment(p>0.05). The Logistic rgression analysis showed that there were significant differencein grave cerebral edema and pneumonia between the2groups. According to the ordersof OR values they were: grave cerebral edema(OR=5.85195%CI=1.589~21.547);pneumonia(OR=11.17195%CI1.198~104.165).Conclusion1.The peak age in all of cases is50~59year.2.The main factors affect the outcome of MBI are grave cerebral edema andpneumonia.
