Objective: to evaluate the role of ischemia modified albumin (IMA) and ECGchanges within half an hour in the early diagnosis of acute coronary syndromes(ACS).Methods:59patients suspected of ACS within3hours were selected byexperimental group. Blood samples of all patients were collected at immediately,3hand24h after Admission, and were detected IMA, troponin I (cTnI) and creatinekinase-MB(CK-MB). Review ECG changes after admission instantly and15-30minutes and do a12-lead ECG.18-lead ECG were done in patients suspected of rightventricular myocardial infarction and posterior wall myocardial infarction(combinedwith posterior wall and right chest leads).Observe two ECG changes and compare thetesting results of IMA, CK-MB and cTnI in different time.Analysis the value ofabove indexes of acute chest pain patients in ACS and discuss the role of IMAcombined with ECG changes in early differential diagnosis of chest pain patients.20healthy physical examine selected by the physical examination center of The Secondaffiliated Hospital of one University were taken blood by health contrast group to testIMA normal reference value.Results:59patients selected patients, the final diagnosis AMI6person, UA26person, NICP27person. Less than three hours of ACS,3~6hours IMA a significantlyhigher level, and NICP group and health physical examination group there existsignificant differences, More than24hours of ACS IMA level NICP group comparedwith no obvious difference. NICP group and health physical examination group IMAno obvious difference. UA group and the group in each level AMI IMA timecomparative differences was not statistically significant. IMA for3hours of thesensitivity of the ACS diagnosis and Negative Predictive value was87.5%and78.9%, higher than the ECG, CK-MB, cTnI, diagnosis of3~6hours of ACSsensitivity and Negative Predictive value was81.3%and70.0%,Higher thanCK-MB, ECG, cTnI. But IMA IMA relative to the united half an hour ECG changes,diagnostic accuracy rate is low, the difference was statistically significant.Conclusion: IMA is a sensitive biochemical index in the early diagnosis of ACS;the sensitivity of IMA is higher in ACS within6hours(especially3hours).IMAcombined with ECG changes within half an hour may have a more value to earlydifferential diagnosis of ACS.
