Objective To analyse the prognostic factor of patients with rupturedabdominal aortic aneurysm after surgery.Methods40cases of RAAA undergoing surgical treatment in the FirstAffiliated Hospital of Chongqing University of Medical Sciences from Aug1999to Dec2010were analyzed retrospectively by age, gender,comorbidities, diameter of aneurysm, blood transfusion, operation time andduration of hypotension. The relationship of these variables with mortalitywas analyzed by both univariate and multivariate analyses.Results In the univariate analysis: patients with hypotension(P=0.034)presented with3.2times mortalitycomparedtopatientswithouthypotension. And patients with duration of hypotension≥2h(P=0.008)presented with2.6times mortality compared to patients with duration ofhypotension<2h. An increased mortality was observed in patients withcoronary heart disease (P=0.028), chronic obstructive pulmonary disease(P=0.012) and renal dysfunction (P=0.028).And an increased mortality wasobserved in patients older than70years(P=0.031)and in diameter of aneurysm bigger than5cm(P=0.016). The relationship of mortality to gender,blood transfusion and operation time was not significant. In the multivariateanalysis: the test showed as significant as age (P=0.040),patients withcoronary heart disease(P=0.036) and duration of hypotension (P=0.042)but not diameter of aneurysm (P=0.225) and other disease.Conclusion E1der, or patient with coronary heart disease andduration of hypotension≥2h had worse prognosis. Shorten the duration ofhypotension can significantly improve the prognosis of patients withruptured abdominal aortic aneurysm.
