Aims To compare association of lipid accumulation product (LAP)and waist circumference (WC) with insulin resistance (IR) in women withpolycystic ovary syndrome.Methods121PCOS patients and115age-matched controls wereenrolled. Anthropometric index(including hight, body mass, WC) andfasting serum metabolic parameters(including lipid profile, plasma glucose,totol testosterone and high sensitive C reactive protein levels et al) weredetermined. An oral glucose tolerance test with75g glucose was performedon all subjects.90PCOS subjects and20controls underwenteuglycaemic–hyperinsulinaemic clamp to assess their insulin sensitivity,which was expressed as M value. The HOMA2-IR and LAP[(WC-58)*TG]were then calculated.Results Compared with control subjects, PCOS women hadsignificantly higher LAP and WC levels and decreased insulin sensitivity (P<0.05), Subgroup analysis showed that compared with age-andWC-matched controls, PCOS women still had decreased insulin sensitivityand increased LAP levels (P<0.05); Both LAP and WC are correlated withM value (r=-0.68, r=-0.66, P<0.01) and HOMA2-IR (r=0.55, r=0.49,P<0.01), and after adjusted for WC LAP still correlated with Mvalue(r=-0.37, P<0.01) and HOMA2-IR (r=0.26, P<0.01). Stepwise logisticregression analysis revealed that only LAP rather than WC could enter themodel as a significant variable in IR assessment. Binary logistic regressionanalysis revealed that the risk of IR in high LAP group was6.05times ofthat in the normal LAP group [Odds Ratio (95%CI):6.05(1.67,21.90),P<0.05].Conclusion In women with PCOS, LAP is associated with insulinsensitivity and it has a better correlation with IR than WC.
