Objective: To compare and analysis the change of neck-shaft angle andshortening of femoral neck after intramedullary fixation of intertrochantericfracture of femer, also to investigate the advantage of intramedullary fixation inintertrochanteric fracture especially in unstable intertrochanteric fracture.Method: Collect the general information and radiographs of patients withintertrochantric fracture who were admitted in PLA general hospital from2009to2012retrospectively, compara the change of neck-shaft angle and shortening offemoral neck after different kinds of fixation of these fractures. Finally,159patientsin PLA general hospital were included. All the patients received a standard surgery,compare and analysis the information and the radiographs before during and after thesurgery, reslice the CT images of different times after surgery using MIMICSsoftware, observe the stabilization of the fracture site4months after treated byintramedullary fixation, analysis the stabilization condition of the fracture site afterintramedullary fixation of the intertrochanteric fracture.Result:In the retrospective study a total of159patients were screened forinclusion. Patients were ranked to be the satble fracture and unstable by AO system.The difference of the mean change of neck-stem-angle and the length of neck for thefracture abuting on bone (type1) and abuting on nail (type2) are not abvious(P>0.05), however, in unstable fracture, it’s obvious (P<0.05).Conclusion:It’s suggested that for stable fracture, the stability of fracturefragment abutting on bone and fragment abutting on nail are similar, but for unstablefracture the stability is different. The fracture abutting on bone is more stable. For ITfractures, intramedullary fixation can get better stability, especially for unstable ITfractures, intramedullary fixation can be better than extramedullary fixation toprevent the collapse and secondary loss of reduction of the fracture site.
