Objective:①Clinical observation the effect of Laifuchengqi Tang of acute lung injury related to severe acute pancreatitis.②At the same time, to research the severe acute pancreatitis models in rats, In the application of treatment on the basis of Sandostatin, add with irrigation stomach, Through test the content of MDA and SOD in the lung homogenate, observation of the protective effects of Laifuchengqi Tang to the lungs. And to further explore the lungs to the protection system of Laifuchengqi Tang.METHODS:①61cases of severe acute pancreatitis patients were randomly divided into the control group and Chinese medicine treatment group, Chinese medicine treatment group was based on conventional treatment to pile Laifuchengqi Tang enema, two groups of patients, observed the lung function, arterial blood gas analysis index, blood amylase, urine amylase, body temperature, white blood cells count, exhaust time, first to defecate time and APACHE II score changes.②Sixty Wistar rats were randomly divided into5groups:control group, SAP model group, Sandostatin group, Laifuchengqi Tang group, combination of Sandostatin and Laifuchengqi Tang group, Each group of each12only. SAP model was induced by retrograde infusion of5%Sodium Cholate into the bile-pancreatic duct. The rats in control group only were fliped intestine several times and needn’t retrograde infused Sodium Cholate. Oerteotide was subcutaneous injection30min after SAP was induced. Laifuchengqi Tang (25ml/Kg)was intragastric administrated8hours and16hours after SAP was induced. Record artery blood gas24hour after operation. Measurement blood amylase, The wet to dry lung weight ratio and the pathological change in lung and pancreatic was recorded. Take part of the lung tissue, processed into the lung homogenate, measurement MDA, SOD content.Results:①The same time Chinese medicine treatment group lung function index, blood gas analysis index and APACHE II score is obviously superior to conventional control group(P<0.05); body temperature, white blood cells count and restore the exhaust defecate time to the control group significantly shortened(P<0.05).②SAP model rats, plasma endotoxin level is blank control group increased significantly, SOD level declined obviously, MDA a significantly higher level. Sandostatin rats to SAP serum amylase level and pancreas pathological score significantly improve, SOD and MDA and so on various index no change. Laifuchengqi Tang group rats, plasma endotoxin level declined obviously, and lung tissue SOD vigor significantly higher, MDA content decreased obviously, lung wet dry weight ratio, arterial blood gas index and lung tissue pathology scores are a SAP rats, obviously improved. Share group of each index is obviously better than single drug, rat models of24h mortality rate decreased obviously.Conclusion:SAP often combined acute lung injury, clinical observation and animal experiments both confirmed Laifuchengqi Tang can greatly improve severe acute pancreatitis lung injury situation, its mechanism may inhibit the lung tissue with local oxidative stress reaction and improve excessive on endotoxin blood disease. In the use of Sandostatin inhibition of pancreatic enzyme combined application of Laifuchengqi Tang treatment, the effect obviously better than single drug, for integrated traditional Chinese and western medicine in clinical treatment with SAP merger ALI to provide the theory basis.
