ObjectivesTo study the effect of the patients who use TR Band or Tian Qi Spinning radial artery tourniquet after the transradial coronary intervention, find out the situaton of patients locality uncomfortable and complications when they use radial artery tourniquet; Analyse the complications reason, and find the avoid method; to compare the two type radial artery tourniquets advantage and disadvantage, give advance to clinical.MethodsTwo hundred patients, who inchanged in Medical ward two, TEDA International Cardiovascular Hospital during June2011to2009to December2011by convenience sampling, were randomly assigned to receive either TR Band(Group I) or Tian Qi Spinning(Group II) radial artery tourniquet. Get the effect of the two type radial artery tourniquets, collect patients common data, special data,(such as blood pressure, heart rate, SPO2), patients local comfort level and local complication. Then use one-way ANOVA, two independent samples t test, Chi-square test and so on analyze the data, compare and analyze the effect of the two type radial artery tourniquets.Results1. Two hundred patients who suffer from Cardiovascular disease (CAD),128patients (64.0%) get Hypertension,113patients (56.5%) get Hyperlipedemia,41patients (20.5%) get Diabetes Mellitus,104patients (52%) has smoking history,138patients (69.0%) is male, the mean age is57.82±6.02, the mean BMI is24.67±2.37kg/m2, these dates are prove that CAD is closely relate to Hypertension, Hyperlipedemia, Diabetes Mellitus, smoking, sex and age.2. None patients in the two groups changed the radial artery tourniquet because of couldn’t hemostasis. 3. None patients in the two groups has oozing(leakage of blood from puncture site requiring digital pressure) after remove the tourniquet.4. There are no differences between two groups concerning SPO2in before TRI and TRI2h later, and the SPO2is normal(Group I t=1.016, P>0.05, Group II t=1.922, P>0.05).5. Local complications were assessed after the removal of compression device, and there are no differences between two groups concerning skin lesion, ecchymosis, local hematomas, Radial artery occlusion(RAO). But the location and nature of skin lesion are different. Most skin lesions in Group I occoured at the bone tower part, patient feeled little pain; in Group II, most skin lesions occoured under the pressure plate, patients feeled very pain and take a long time to recover.6. Concerning the comfortlevel, Group I patients feel numb and swell werse than Group II, Group I patients feel I werse pain than Group I.The times Groupe I call help is more than Group II.7. Patients get higher SBP and HR at the time before going to the operation room than2h later after TRI, but the DBP has no difference.ConclusionWrist circumference and BMI is positive correlation. TR Band or Tian Qi Spinning radial artery tourniquet are effective radial artery tourniquet. There are no differences concerning local difference, but Tian Qi Spinning radial artery tourniquet can cause bad skin lesion, needing doctors and nurse special attention; there differences concerning about comfort level, most TR Band radial artery tourniquet local discomfort is numb and swell, special for the patients with big wrist circumference; most Tian Qi Spinning radial artery tourniquet is pain under the pressure plate, maybe the pressure plate is’nt enough elastic.Patients get higher SBP and HR at the time before going to the operation room than2h later after TRI, showing patients are anxiety, pointing that doctors and nurses should pay attention to patients psychology care.
