Objective To study the effects of the spinal stability after reconstructionusing the screw-rod system with posterior resection of intraspinal tumor andlaminectomy.Method Retrospectively analysed29cases with pathologicallyconfirming as intraspinal tumors from May2008to December2011in the firstaffiliated hospital of Guangxi medical university, using the comparative analysisof preoperative and postoperative. Among these cases,18males,11females,aged from10years old to72years old, the average of38.5. All cases werefollow-up from3months to42months, the average of13months. All patientstook X-ray, MRI, partly CT scan. Preoperative MRI suggested it’s all epiduraltumors, and the tumors’ diameters are between2.0cm and18.0cm, the averageof5.0cm. All patients underwent posterior laminectomy and took the screw-rodsystem fixation and partly bone grafting. The main outcome records:complications of the surgery、pain relief、recovery of neurological function、quality of life、review of the X-ray. Pain relief assessed by visual analogue scale,recovery of neurological function is in accordance with Frankel grade, andquality of life assessed by the quality of life assessment.Results Pathological examination shows:20cases of schwannoma, one case of ependymoma,2cases of neurofibromatosis, one case of lipoma, one caseof hemangioma, one case of chondroma,3cases of cysts. Complications: onepatient had early localized infection, one patient had cerebrospinal fluid leakage,the others’incisions healed well.2Cases were found as malpositioned, and2cases of screw beyond the anterior of the vertebral, but not exceeding5mm.There is no injury of large blood vessels、nerves and spinal cord. For all cases,the pain disappeared or reduced, compared a preoperative average of5.8pointswith a postoperative average of0.86points, which have significant difference (P<0.01). The grade of neurological function by frankel grade: preoperation wasgrade C3, grade D24, grade E2and that turned to0,5,24after three monthspostoperation, which had significant difference (P <0.01). The quality of life:using the quality of life assessment, it is64.7points before surgery, comparedwith89.1points, three months postoperation, which had significant difference(P <0.01). Observation of the X-ray films that were taken by three monthspostoperation showed a good stability of the spine structure, and the screws androds reign were well fixed. There’s no broken and loosening of the screws androds, and vertebral gap is not narrowed. The planting bone partly began tofusion.Conclusion With the strict indications, on the basis of posteriorlaminectomy, interbody fusion and posterior screw-rod system fixationtechniques can be effective to reconstruct the spinal stability, and available ofgood treatment. It is an effective method to prevent the emergence of spinaldeformity, and the long-term effect needs further consecutive follow-up.
