Objectives To investigate the incidence, etiology and the prognosis ofacute kidney injury(AKI) among hospitalized patients in First AffiliatedHospital of Guangxi Medical University, and provide a basis for the preventionand treatment of diseases.Methods Patients who were admitted in the First Affiliated Hospital ofGuangxi Medical University from March1st2011to August31st2011wereinvolved. After checking the computer-based data on kidney function, patientswith acute kidney injury were picked out and further history reviews weredemanded to get the information of clinical characteristics, prognosis and thecauses of AKI.Results There were28291admissions during the study period and141met with the diagnostic criteria of acute kidney injury (AKI). The overallincidence of AKI was0.50%. In-hospital mortality was0.78%in all dischargeswhile19.86%in patients with AKI. And infection (18.44%in AKI)anddrugs(16.31%in AKI) were the main causes of parenchymal AKI(52.48%inAKI). The low heart output(32.62%in AKI)was the main cause of the prerenal AKI(45.39%in AKI). The prerenal AKI(62.90%in HA-AKI)especially the lowheart outpu(t56.45%in HA-AKI)was the main cause of hospital-acquired acutekidney injury. The operation was35.48%in HA-AKI, and the operation of heartand vessel (25.81%in HA-AKI) was especially apparent. Renal hypoperfusion(21.95%in A on C) and application of renal damage drugs(21.95%in A on C)were the main predisposing factors of acute kidney injury on chronic kidneydisease. The results of logistic regression analysis suggested the multiple organdysfunctional syndrone(OR17.379)and not using diuretics (OR3.814)were therisk factors of death among AKI patients. And the result of logistic regressionanalysis suggested the hospital-acquired (OR0.331)was the influence factor ofcure rate.Conclusions1. Acute kidney Injury is prevalent in hospitalized patients,and the mortality of AKI is high.2. The parenchymal factor and the prerenalfactor are the main causes of AKI.3. The low heart output is the main cause ofthe prerenal AKI. And drugs is the main causes of parenchymal AKI.4. The lowheart output is the main cause of the prerenal HA-AKI. And drug is the maincauses of parenchymal HA-AKI.5. The operation especially operation of heartand vessel is the main cause of HA-AKI.6. Renal hypoperfusion and applicationof renal damage drugs are the main predisposing factors of acute kidney injuryon chronic kidney disease.7. The multiple organ dysfunctional syndrome andnot using diuretics are associated with significantly increased mortality.8. Thehospital-acquired is the influence factor of cure rate.
