Objective To explore the clinical significance of serum cystatin C(Cys C) in renal impairment of multiple myeloma, and Cys C level differences among the different TCM Syndrome Kinds.Methods Choose the patients who are accord with diagnosis standard of multiple myeloma. Observation group includes32cases altogether, these patients must been diagnosed as multiple myeloma in Wuhan General Hospital of Guangzhou Military. According to the international ISS staging system, these patients were classified.30patients who are with the normal indicators of outpatient examination were chosen as control group. Statistical data were analyzed with SPSS17.0statistical software, and were denotesed with the mean+standard deviation. Two groups were compared using t-test. P<0.05was considered statistically significant difference. Multiple samples were compared using Student-Newman-Keuls.We use Person correlation analysis to analyze correlation. Patients of observation group were classified according to the four dialectical methods of inspection, listening and smelling examination, inquiry, palpation. We try to find the corresponding relationship between the Cys C level of MM patients above the normal range (impaired renal function) and the different TCM Syndrome Differentiation.Resultsl.Urea nitrogen, creatinine, uric acid, β2-microglobulin and Cys C levels were compared between observation group and control group. The results showed that Cys C levels (1.81±1.47) of observation group was significantly higher than Cys C levels (0.58±0.15)of the healthy control group(p<0.05), and the levels of urea nitrogen, creatinine from observation group were also prompted increased. These were consistent with Cys C levels.2. The observation group includes newly diagnosed multiple myeloma patients. The results showed that the serum Cys C levels of ISS-Ⅲ patients was significantly higher than the serum Cys C levels of ISS—Ⅰ patients and ISS-Ⅱ patients. The result was considered statistically significant difference (P<0.05).The difference between the serum Cys C levels of ISS-I patients and the serum Cys C levels of ISS-Ⅱ patients was no significant difference.3. The difference between the levels of urea nitrogen and creatinine from observation group patients at the first day after chemotherapy and that from observation group patients before chemotherapy shows no significant difference. But difference of the serum Cys C levels between them was considered statistically significant difference. The sensitivity of the serum Cys C levels for the patients with multiple myeloma in renal impairment is higher.4. we use Person correlation analysis to analyze correlation of the the serum Cys C levels and β2-MG from patients of observation group before chemotherapy. The results showed that Cys C and β2-MG was positively correlated.5.32cases of multiple myeloma patients were classified according to Traditional Chinese Medicine. The serum Cys C levels of kidney dificiency blood stasis and kidney deficiency damp-heat was higher than others.Conclusions The serum Cys C levels can be used as a sensitive marker of renal impairment in multiple myeloma, and is better than urea nitrogen,creatinine and uric acid. Among MM patients with renal dysfunction, the majority is kidney dificiency blood stasis and kidney deficiency damp-heat.
