| ObjectivePeritoneal dialysis (PD) is one of the main methods of end stage renal diease of renal replacement therapy. Ultrafiltration,loss of function, has become long-term peritoneal dialysis patients out of one of the main reasons. Therefore, how to improve peritoneal ultraf iltrative function on peritoneal dialysis patients in recent years, many scholars study the main direction. To explore the issues of Shenxiong Glucose injection on patients with peritoneal dialysis peritoneal ultrafiltration function effect, whether is the relationship of the carbohydrate antigen CA125and peritoneal ultrafiltrative function correlation or not.Method1. This topic observed objects are from Wuhan to combine traditional Chinese and Western medicine hospital kidney internal medicine department peritoneal dialysis center from June to December in2011 between inpatient and outpatient dialysis patients, parallel continuous ambulatory peritoneal dialysis is greater than three months in peritoneal dialysis patients,a total of sixty.2. The clinical observation ofpatients are randomly divided into two groups,control group and treatment group.Among the twenty-four cases of the control group, which is fourteen male cases and ten female cases, aged from twenty to sixty-five years old, average age is51.4±11.7years; peritoneal dialysis age is from six to sixty-four months, average peritoneal dialysis age is23.8±18.6months; primary disease is that chronic glomerular nephritis is fourteen cases, diabetic nephropathy is four cases, hypertensive nephropathy is four cases,obstructive nephropathy is one case and chronic interstitial nephritis is one case.The treatment group of thirty-six cases, which is twenty-two male cases and fourteen female cases, age from twenty to sixty-five years old, average age is51.6±11.9years; peritoneal dialysis age is from six to sixty-four months, average peritoneal dialysis age is23.7±18.8months. Primary disease is which chronic glomerular nephritis is senventeen cases, diabetic nephropathy is eight cases, hypetensive nephropathy is eight cases, obstructive nephropathy is two cases and chronic interstitial nephritis is one case. 3.The control group is treated with CAPD for seven days:daily dialysis2L x (3-4) cycles, each cycle of dialysis is poured into the dialysate in peritoneal dialysis patients with2L, which is in the same condition and at the same time along with other conven-tional drugs such as symptomatic treatment. Patients daily must record which contains peritoneal dialysis ultrafiltration volume, urine volume and blood pressure.4.The control group before treatment do standard CAPD therapy in patients with peritoneal dialysis, to maintain the original plan. In the morning of the first day is to be line standard peritoneal equilibration test and take overnight dialysate to concentrations of CA125. In the seventh day that is to repeat the PET examinations and take overnight dialysate effluent.5. The cases in the control group on the basis of Shenxiong glucose injection, to50ml/2L add to peritoneal dialysis solution of seven days, every patient still mains the original dialysis regimen, line of standard CAPD treatment, the rest of the control group.6. Line standard peritoneal equilibration test is that before test patients do standard CAPD therapy. In the morning of the first day in test it is collecting24h effluent and24h urine and weighing metering, then patients who take2.5%glucose of peritoneal dialysis fluid of2L injected into the peritoneal cavity is at the speed of 200ml every minute in ten minutethat. In addition the patients must change one position when every400ml peritoneal dialysis fluid is injected into the peritoneal cavity.Then taking out Oh and2h in the effluent from each200mland leaving10ml effluent and10ml urine immediately inspection. After four hours patients take seat with20min taking out the effluent, and leaving20ml dialysate. Daily ultrafiltration volume=24h dialysate effluent total amount-24h peritoneal dialysis.7. The biochemistry targets that are effluent CA125concentration,24h urea nitrogen, creatinine and24h urinary of urea nitrogen, creatinine. Oh,2h and4h urea nitrogen, creatinine, blood creatinine (Scr), blood urea nitrogen (BUN), hemoglobin (Hb), plasma albumin (Alb), calcium(Ca), phosphorus (IP), parathyroid hormone (PTH).8.The application of SPSS17.0software the analysis results, the data using the mean and standard deviation (x±s) representation, comparison group by group t test data. Comparison group in group is testing using self controlled paired test.Result1. The will of Shenxiong Glucose Injection in peritoneal fluid seven days after treatment, group therapy for patients with ultrafiltra-tion volume by prior treatment with320±65ml/d wereincreased to662±85ml/d, compared with those before treatment was statistically significant (P<0.05), while the control group before treatment volume310±60ml/d after treatment, the ultrafiltration volume315±63ml/d, before and after treatment show no significant change(P>0.05). The two groups after treatment,the treatment group than the control group with significant difference (P<0.05)。2. The will of Shenxiong Glucose Injection in peritoneal fluid seven days after treatment, the therapy group of patients with peritoneal dialysis liquid CA125concentration from14.6±3.2U/ml before trea-tment to17.8±3.6U/ml than before treatment have a very signifi-cant difference(P<0.05),while the control group before treatment CA125concentration14.3±3.4U/ml, after treatment concentration of CA12514.5±3.2U/ml, no significant change (P>0.05), the treat-ment group and the control group after treatment, the treatment group than in control group with significant difference (P<0.05).3. The will of Shenxiong Glucose Injection in peritoneal fluid seven days after treatment, the treatment group before and after treatment in patients with urine volume were not showed no significant change (P>0.05). The treatment group and the control group between the two groups after treatment, patients with urine volume were not had no obvious change (P>0.05).4. The will of Shenxiong Glucose Injection in peritoneal fluid therapy after seven days, the treatment group patients before treatment total KT/V(urea clearance index) of1.88±0.38, total KT/V after treatment wasl.92±0.41, showed no statistical signifi-cance (P>0.05); the treatment group patients before treatment, total Ccr was64.2±15.3L/week, after treatment of total Ccr66.3±16.1L/week, no obvious statistical significance (P>0.05); the treat-ment and the control group after treatment between two groups, total KT/V, total Ccr was not significant difference(P>0.05).5. The will of Shenxiong Glucose Injection in peritoneal fluid seven days after treatment, the treatment group of peritoneal transport function(D/Pcr) before treatment was0.63±0.18, D/Pcr after treat-ment was0.66±0.17, showed no statistical significance (P>0.05). The treatment group and the control group between the two groups after treatment, D/Pcr had no significant change (P>0.05).6. The will of Shenxiong Glucose Injection in peritoneal fluid seven days after treatment, the treatment group before and after treatment in patients with the level of hemoglobin (Hb), plasma albumin (Alb), calcium (Ca), phosphorus (IP), parathyroid hormone (PTH) had no signi-ficant change (P>0.05). The treatment group and the control group between the two groups after treatment, patients hemoglobin (Hb), plasma albumin (Alb), calcium (Ca), phosphorus (IP), parathyroid hormone(PTH) showed no statistical significance (P>0.05). 7. The will of Shenxiong Glucose Injection in peritoneal fluid seven days after treatment, the treatment group in the treatment of patien-ts with blood pressure levels did not change obviously,there are no allergy,abdominal pain so on and other adverse reactions.Conclusion1. Peritoneal dialysis patients use Shenxiong glucose injection of peritoneal dialysis fluid in CAPD therapy, which can increase the amount of ultrafiltration in the short term.2. In peritoneal dialysis patients with Shenxiong Glucose Injection for treatment of peritoneal dialysis peritoneal fluid, peritoneal fluid drainage liquid CA125concentration increasing which shows Shen xiong glucose injection may be a protective role of peritoneal mesothelial cells.3. Shen xiong glucose injection of safetyis good, the short term on CAPD patients on peritoneal transport function, solute clearance rate, residual renal function, blood pressure level was not signifi-cantly influence. |
