Objective: Connective tissue disease induced interstitial lung disease(CTD-ILD) patients with connective tissue disease induced interstitial lungdisease (IPF+NSIP) patients and normal human bronchial alveolar lavagefluid in tumor necrosis factor-α (TNF alpha) and transforming growthfactor beta1(TGF beta1) level difference, comparative analysis of differentmeanings, in order to provide a choice of clinical differential. Methods: with the random method, incorporated in2010June-2011Mayin Sichuan Provincial People’s Hospital during inpatient and outpatient visitsin49cases, collecting specimens of bronchoalveolar lavage, divided intothree groups, including:1、13cases of connective tissue diseases in patientswith interstitial lung disease (CTD-ILD), including6cases of systemiclupus erythematosus (SLE) in patients with interstitial lung disease(SLE-ILD group),5patients with rheumatoid arthritis (RA) in patients withinterstitial lung disease (RA-ILD group),2patients with scleroderma (SSA)in patients with interstitial lung disease (group SSA-ILD), as the experimental group;2、12cases of connective tissue disease caused byinterstitial lung disease (IPF+NSIP), including10cases of idiopathicpulmonary interstitial fibrosis (IPF group),2cases of nonspecific interstitialpneumonia patients (group NSIP), as compared to the control group;3、21cases of normal subjects (group control), as the normal control group. The three groups were using specific enzyme-linked immunosorbent assay(ELISA) determination of bronchoalveolar lavage fluid in tumor necrosisfactor-α(TNF alpha) and transforming growth factor beta1(TGF beta1)level, compare between groups and statistical analysis, the two kinds ofcytokines combined detection in bronchial bronchoalveolar lavage fluid incontent and its significance. Results: Connective tissue disease in patients with interstitial lung disease(group CTD-ILD) in bronchoalveolar lavage fluid tumor necrosis factor-α(TNF alpha) and transforming growth factor beta1(TGF beta1) levelswere23.05+5.52pg/ml,165.44+37.59pg/ml; non connective tissuedisease in patients with interstitial lung disease (IPF+NSIP) group ofbronchoalveolar lavage fluid in tumor necrosis factor-α(TNF alpha) andtransforming growth factor beta1(TGF beta1) levels were17.01+5.82pg/ml,132.46+23.45pg/ml; normal controls in bronchoalveolar lavage fluidtumor necrosis factor-α(TNF alpha), human transforming growth factorbeta1(TGF beta1) levels were6.82+3.97pg/ml,46.03+11.83pg/ml.Connective tissue disease in patients with interstitial lung disease (group CTD-ILD) in bronchoalveolar lavage fluid tumor necrosis factor-α(TNFalpha) and transforming growth factor beta1(TGF beta1) levels weresignificantly higher than that of the control group, the difference wasstatistically significant (P <0.05). Non connective tissue disease in patientswith interstitial lung disease (IPF+NSIP) in bronchoalveolar lavage fluidtumor necrosis factor-α(TNF alpha) and transforming growth factor beta1(TGF beta1) levels were significantly higher than that of the control group, the difference was statistically significant (P <0.05). Connective tissuedisease in patients with interstitial lung disease (CTD-ILD) group ofbronchoalveolar lavage fluid in tumor necrosis factor-α(TNF alpha) andtransforming growth factor beta1(TGF beta1) level was significantlyhigher than that of connective tissue disease induced interstitial lung disease(IPF+NSIP), differences there were statistically significant (P <0.05).Conclusion: bronchoalveolar lavage fluid in tumor necrosis factor-α(TNFalpha) and transforming growth factor beta1(TGF beta1) level differencescontribute to the diagnosis of interstitial lung disease. The connective tissuedisease induced interstitial lung disease (CTD-ILD) in the differentialdiagnosis also have tips significance.
