The EAM Clinical Characteristics Analysis Of200AS Patients

Objective:The discovery of the extra-articular manifestations of clinical features in ankylosing spondylitis (AS) patients, and the improvement of diagnostic levels in the patients of AS with extra-articular manifestations.Methods:A retrospective analysis of the articular clinical feature and extra-articular manifestations was made among200patients of AS hospitalized from2007to2011in the Second Xiangya Hospital of Central South University.Results:(1)In200cases of hospitalized patients with AS were132males, mean age24.32±5.02and68females, mean age36.64±3.42.(2)The males with extra-articular manifestations were102cases for (51%), and females31cases accounted for15.5%; the section of the outer performance of AS patients mean disease duration (356.38±12.24months) than those without extra-articular manifestations in the course of the disease (160.42±3.24months) long (P<0.05).(3)The incidence of the low back pain was98cases of male patients accounted for74.2%and44female cases accounted for64.7%.(4)Fever in20cases were accounted for10%and hematological system had been involved in148cases for74%. (5)With ocular uveitis of38cases were accounted for19%, there were29males for76.31%and9females for23.69%, which were accompanied by the increase of platelets.(6)The patients complicated with cardiovascular diseases were42cases accounted for21%, of which3cases were with aortic insufficiency accounted for1.5%and two or three tricuspid regurgitation were in7cases for3.5%.(7)Pulmonary lesions were20cases for5%(8)Patients with renal diseases were29cases accounted for14.5%, of which were4cases of hematuria and20cases of proteinuria including5cases of renal biopsy showed IgA nephropathy.(9)Complicated with nervous system involvement were8cases for4%, showed a weakness, muscle atrophy and line electromyography which suggested peripheral nerve injured.(10)Sternal compression fracture were of3cases.(11) About ESR CRP of laboratory data in the patients of AS with and without extra-articular manifestations, the difference had statistical significance (P<0.05), while HLA-B27had no significant difference (P>0.05).Conclusion:(1)The section of the outer performance of AS patients are more men than women compared to the patients of AS with and without extra-articular manifestations, which are the longer duration and the higher levels of ESR, CRP and the count of the PLT(2)With extra-articular manifestations as the initial symptoms of patients with AS, should strengthen multidisciplinary collaboration, achieve early diagnosis and to avoid missed diagnosis. At the same time, clinical treatment combined with extra-articular manifestations in AS patients should be more active.(3)The mechanism of AS with extra-articular system diseases need to be further defined.
