Clinical Research On Platelet Associated Antibodies In Adult Patients Of Primary Immune Thrombocytopenia

Objective:With FCM,we detectd the expression level of PAIg in adult patients ofimmune thrombocytopenia before and after treatment;To explore the effection of PAIgin the pathogenesis of ITP and their clinical value.Method:(1)FCM was applied to detect the expression level of PAIg in peripheral bloodin19normal cases and47adult patients of ITP before and after treatment;(2)Analysisthe effection of PAIg in the pathogenesis of ITP and their clinical value in diagnosis ofITP;(3)Combinated with the condition and curative effect of patients,analysis theeffection of PAIg to curative effect and prognosis of ITP.Result:(1)The expressions of PAIgA,PAIgM,PAIgG (11.17±7.36%、16.90±10.53%、23.42±16.85%) in the group of patients with ITP was obviously higher than theirs ingroup of healthy persons(1.39±0.41%、4.96±1.04%、5.70±0.84%)(p<0.05),the level ofPAIg was associated with ITP and can be used in diagnosis of ITP;(2) The expressionsof PAIgA,PAIgM,PAIgG (14.24±8.96%、23.92±13.63%、35.51±20.26%) in the group ofserious patients with ITP was obviously higher than theirs in group of mild patients withITP (9.26±5.51%、12.55±4.24%、15.92±8.07%)(P<0.05), the level of PAIg wasassociated with severty of ITP;(3)In effective group,the expressions ofPAIgA,PAIgM,PAIgG after treatment (1.54±0.64%、5.61±1.74%、6.03±1.17%) wasobviously lower than theirs before treatment (10.33±5.46%、13.10±3.94%、16.27±4.99%)(P<0.05),there was no difference compared with the expressions in the group of healthy persons(P>0.05).In uneffective group, there was no difference in theexpressions of PAIgA,PAIgM,PAIgG between after treatment (10.93±8.47%、21.00±13.13%、23.96±19.24%) and before treatment (12.80±10.11%、24.27±14.87%、37.27±22.61%)(P>0.05);The decline of the level PAIg was associated with the curativeeffect of ITP.(4)Before treatment,the expressions of PAIgM and PAIgG in theuneffective group (24.27±14.87%、37.27±22.61%)were higher than theirs in effectivegroup(13.10±3.94%、16.27±4.99%)(P<0.05),there was no difference in the expressionof PAIgA between effective group (10.33±5.46%) and uneffectivegroup(12.80±10.11%)(P>0.05). The levels of PAIgM and PAIgG may be associatedwith the prognosis of ITP.Unit detection of PAIg,especilly the levels of PAIgM andPAIgG,was helpful for calculating in curative effect and prognosis of ITP.Conclutions:(1)ITP was closely related to humoral immunity disorder,PAIg was thekey of the pathogenesis of ITP;(2)The level of PAIg was helpful for the diagnosis of ITP;(3)In the beginning of ITP,the level of PAIg was associated with the disease severty;(4)The levels of PAIg in beginning of ITP,especially the levels of PAIgM and PAIgG,wereclosely related to curative effect, they can be used to calculating the curative effect andprognosis of ITP.
