Expression And Clinical Significance Of HTERT In Hepatocellular Carcinoma

Objective:To investigate the expression of hTERT in hepatocellular carcinoma tissues, liver cancer early detection, treatment options, prognostic analysis provides the basic theory.Research methods:(1) hTERTmRNA transcript levels was deteced by reverse transcription-PCR in hepatocellular carcinoma and pericarcinomatous cirrhotic liver tissue and normal liver tissue;(2) hTERT protein expression amount was deteced by immunohistochemistry method in hepatocellular carcinoma and adjacent liver cirrhosis tissue and normal liver tissue;(3) In clinical level to compare hepatocellular carcinoma image diagnosis examination and hTERT examination in the diagnosis of hepatocellular carcinoma with positive rate;(4)18patients were followed up,whom were detected with hTERT expression in the liver tissue, the follow-up period is12months, took B ultrasound, CT, MRI examination every3months according to the need for patients. Such as the appearance of liver metastases, it followed that termination of follow-up, to see the effect of hTERT expression and hepatocellular carcinoma recurrence relations.Results:(1) hTERT protein positive expression was mainly localized in the cytoplasm, occasionally intranuclear staining, for cytosolic brown granular or patchy staining, positive cells showed diffuse small nests or scattered. There were41cases and18cases of hTERT protein expression in50patients with hepatocellular carcinoma tissue and pericarcinomatous tissue respectively, the two groups had a very significant difference (P<0.01). And21cases with hepatocellular carcinoma tissue was strongly positive (+ ++)in hTERT protein expression,and15cases (++),5cases (+),9cases of (-). In50cases with the corresponding noncancerous liver tissue, there were8cases of atypical hyperplasia of the liver cells in the area of positive signals, but its positive strength was significantly lower than the corresponding cancer tissue, of which2cases accompanied periportal or fibrous septum in a small number of mononuclear cells showed weak positive (+), and the other2were only in the the district or fibrous septum of mononuclear cells in a weak or moderate intensity of expression (++). In48cases of normal liver tissue blank control group showed no positive expression.(2) In50cases of hepatocellular carcinoma tissues, there were41cases positive expression of hTERTmRNA,while there were15cases expression in the noncancerous liver tissue near the HCC tisue, the two groups has a significant difference (P<0.01).There was no case of hTERTmRNA expression in48cases of normal liver tissue by control.(3) There were15cases with hTERT protein expression in20patients with small HCC tissue,the positive rate was75%; and there were15cases with hTERT protein expression in18cases with large liver cancer tissue,the positive rate was83.3%; and in12cases of huge liver cancer tissue,hTERT expression was11patients, positive rate was91.7%. In20patients with small HCC, which imaging diagnosis could find was12cases, the positive rate was60%; and in18cases with large HCC, imaging finded was17cases, the positive rate was94.4%; while in12patients with huge HCC,imaging fined was11patients, the positive rate was91.7%. So the hTERT is superior to imaging diagnosis in the positive rate of diagnosis in small HCC (P<0.05),and the positive rate of hTERT and imaging diagnosis has no significant difference (P>0.05).(4) The positive rate of hTERT in hepatocellular carcinoma and its adjacent tissues was82.0%and36.0%respectively. The differences of hepatocellular carcinoma tissue and paracancerous tissue in positive expression rates have significance by statistical analysis (P<0.05); And the positive rates of hTERT and imaging diagnosis in hepatocellular carcinoma tissue and paracancerous tissue has significant difference.(P <0.01).(5)7incident cases of cancer metastasis during the follow-up period of the total18case (38.9%),4cases of the7incident cases are the strong positive activity of hTERT,2cases are the positive activity of hTERT,1case is the weak positive activity of hTERT, no case of cancer metastasis happened in the group of negative activity of hTERT. hTERT has heralded the recurrence and metastasis of hepatocellular carcinoma.The stronger activity of the hTERT, the more recurrence and metastasis of hepatocellular carcinoma may be took place.but for the postoperative recurrence of hepatocellular carcinoma research, the subject of the follow-up of the patient cases is insufficient, we need to increase the amount of sample further.Conclusions:(1) hTERT in hepatocellular carcinoma tissues has a more higher expression levels than that in tissues adjacent to HCC, normal liver tissues;(2) hTERT,in hepatocellular carcinoma diagnosis:the positive rates of hTERT is higher than that of imaging diagnosis in small HCC, and there are no differences in large hepatocellular carcinoma and huge hepatocellular carcinoma diagnosis; In the diagnosis of tissue adjacent to carcinoma, hTERT is superior to imaging diagnosis.(3) hTERT has heralded the recurrence and metastasis of hepatocellular carcinoma, which maybe become a valuable marker for diagnosis and prognosis of HCC.
