Objective:To investigate the value in diagnosis of ovarian cancer recurrence through diffusion-weighted imaging.Methods:DW images and MR images were performed in28cases that were confirmed by pathologic results or follow-up results from2011-06to2012-05. All images are achieved by1.5T GE MR with body coil with two b-values (0and800seconds/mm2).All the cases were evaluated in blinding. The abdominal cavity and the pelvic cavity were divided into five parts for evaluation. We do the analysis of lesion distribution in the abdominal cavity and pelvic cavity abnormalities. The diagnosed indexes and the receiver operating characteristic (ROC) curve analysis are used to do the analysis to the value of DW imaging in diagnosis. The shape, size, and signal change of lesions before and after treatment of the lesions are also evaluated.Results:The hot spots of ovarian cancer are the paracolic gutter, bowel serosa, greater Omentum, the right subphrenic space, the Douglas pouch and the pelvic wall. And the lymph nodes force on theIn the ROC analyses, the areas under the ROC curve were0.813,0.873,0.747, and0.753for DWI and T2WI by doctorl and doctor2, respectively. The AUC for four parts were0.853,0.769,0.818,0.767and0.673,0.680,0.493,0.733,and0.898,0.827,0.818,0.767and0.696,0.669,0.653,0.631for DWI and T2WI by doctorl and doctor2, respectively8patients who is suspected in clinical was found107lesions in the DW imaging before the treatment, of which82lesions significantly smaller, disappear or lower signal on DWI after the treatments,30lesions is unchanged. One aggression cases,5lesions were disappeared;15lesions get large,10lesions were unchanged in the shape, size and signal, and8lesions were new. Another two clinical-negative cases with11lesions were followed up,3lesions disappeared,8lesions as same as before.Conclusion:DWI is superior to T2WI in the detection of recurrent ovarian cancer lesions.
