The Study On Status And Persistent Effect Mechanis Of Tobacco Control In Three Counties Of Rural Shandong Province

With the rapid development of China’s socio-economic, the traditional risk factors caused by poverty such as malnutrition, the poor health condition are being gradually replaced by modern risk factors including smoking, alcoholism, and high-fat diet; smoking has become a major risk factor for death. China is the largest tobacco production and consumption country in the world, and the tobacco trend in China is very serious,the rural population is smoking the main population in China.Researching the rural prevalence of smoking and drafting the tobacco control measures and policies in rural areas have an important strategic and practical significance to improve the quality of rural life and health standards, and also can promote the construction of a new socialist countryside.ObjectivesAnalyze tobacco epidemic and its influence factors, as well as the present situation of tobacco control (TC) and the existing problems by investigating in three rural counties of Shandong province; Build long-term effective mechanism of TC in rural areas, so as to improve the health level of the massive rural population and the quality of life.Data Resource and MethodThe data of this study came from the Tobacco Control survey of the health research base of Shandong University (Pingyin County, Liangshan County, and Junan County). Individual interviews and focus discussions were used to gather information.Totally58people received the personal interview,109people attended the seminar.This study sorted all the collectted dataes, and used qualitative analysis software Nvivo8.0to analysis these dataes.Main Results(1) The tobacco epidemic and reasons of smoking in three counties of rural Shandong provinceThe smoking rate of the three counties rural male residents is75.69%, among them over the age of40.;Smoking rates of health agency in three counties was40%,interviewers found leader in health institutions often smoke; Education Bureau with less number of smoking, smoking pheonmenon of teachers from junior middle school are more than teachers from senior high school and primary school in the three counties.,middle and high school students have smoking phenomenonSocial needs and big work pressure are the main smoking reasons of the research objects; Rural residents spent their free time by smoking in three counties; Being curious about smoking was original reason for students and the parents had a major impact on their smoking.(2)The status of TC in three counties of rural Shandong province1) Formulation of tobacco control policies:health agencies of the three counties developped tobacco control implementation plan,but lack of TC policy from government, leaders’ smoking behavior affect the implement of tobacco control policy.2) The organization form of TC:health agency of TC in the three counties set up tobacco control leadership team, and confirm the organization and responsibility, but short of TC execution power due to the shortage of TC measures.The three counties were out of TC organization network to cover all people of the three counties.3) Activities and effect of TC:TC activities in the three counties included smokeless disease control activities, no tobacco day campaign, health curriculum, public health worker advising sufferers of tobacco control ect. The TC activities of three counties had made certain effect, but the activities involved coverage,especially rural resident are less, and atmosphere of TC is thick, and it didn’t attain the due effect.(3) Persistent effect mechanism of TC in rural areas1) Organization management of TC:three level of tobacco control network of county-town-village should be established, and confirm the extent and responsibility of TC.2) The system construction of tobacco control:perfecting TC system of rural areas, including TC measures for the management inrural areas, training system of TC,publicity system of TC, evaluation and supervision system of TC, and awards and penalizations ystem of TC ect.3) Safeguard measures of TC:TC needs powerful support of leaders, capital guarantee of TC and monitoring exposure of tobacco, and evalution of tobacco control effect.Conclusions and Suggestions(1) The TC persistent effect mechanism were lacked in three counties rural area. Set up the persistent effect TC Mechanisms of government making TC policies and the leaders taking the lead.(2) The ability of medical personnel in rural is not enough to support TC sevices,improve TC awareness and TC abilities of medical workers, put stop smoking drugs into the drug reimbursement directory of the new rural cooperative medical system.(3) Integrate various publicity channels and media forces to promote TC education work.Improve Cognitive Level of TC health knowledge in the rural areas, develop TC atmosphere and direction of public opinion in rural areas.
