Clinical Study On Dimensions Of Myopectineal Orifice On Living Bodies Of Chinese People

Objective:To evaluate the characteristics of Chinese myopectineal orifice and the correlativity between the body parameters and the myopectineal orifice dimensions, to provide anatomical data for selecting better prosthetic patches of tension-free hernioplasty through measuring and calculating the body parameters and the myopectineal orifice dimensions in Chinese people before and during operation respectively.Methods:This study consisted of a current analysis of cases with inguinal hernia at a single hospital(Jinan central hospital of Shandong university) between January2011and September2011. We currently analyzed the data of83patients(85cases) underwent hernia repairment before and during operation, adding up the patients’ general information, and measuring the patients’height, weight and the distances of surface sites related to pelvis preoperatively. During the operation measurements of myopectineal orifice were made. With the approach of statistics the size of myopectineal orifice accompanied by the correlations between myopectineal orifice dimensions and body parameters (distance of surface sites related to pelvis, height and weight) were researched.Results:The surface area of Chinese myopectineal orifice was15.6±6.6cm2(male16.8±6.0cm2, female8.0±5.3cm2) in our groups while the maximum was41.0cm2and the minimum was3.3cm2. The length of boundary of myopectineal orifice was5.3±1.4cm,4.3±1.3cm,3.2±0.7cm and4.8±1.4cm superiorly, medially, inferiorly and laterally, respectively. The myopectineal orifice averaged5.7±1.9cm in height, and5.6±1.2cm in width. Gender differences in surface area, height and length of lateral boundary were discovered in our study(P<0.05), whereas there was no statistical significance in sexual differences of width and the lengths of boundaries except for lateral one(P>0.05). Significant gender differences were found: in male individuals, myopectineal orifice size, the supraligamental and infraligamental height, myopectineal orifice height and length of lateral boundary (16.8±6.0cm2,2.7±0.7cm,3.4±1.1cm,6.1±1.5cm,5.0±1.3cm) were greater than in females(8.0±5.3cm2,1.6±1.2cm,1.6±1.5cm,3.2±2.7cm,3.4±1.0cm). The median,75th and25th percentile of surface area of male myopectineal orifice was16cm2,19cm2and13cm2, respectively, and that of female was7cm2,11cm2and3cm, respectively. The medians of the lengths of boundaries and vertical dimensions were almost the same as their averages. Myopectineal orifice in males showed the greater height than width(5.5×6.1cm), whereas in females, its width was greater than its height(5.9×3.2cm). Noticeable correlations of the myopectineal orifice parameters to constitutional parameters were found for the two datas(height of body and the pelvis) versus the three parameters(the surface area, height of the myopectineal orifice and length of boundary). However we found no correlations of the three datas(weight, pelvis width and the distances related to the point of femoral pulse) versus the size of the myopectineal orifice(P>0.05). The pelvis height correlated with the size of myopectineal orifice in negative(r<0).Conclusions:Noticeable correlations of the mean surface area of Chinese myopectineal orifice to the height of body and pelvis were found. Significant gender differences were discovered:in male individuals, the surface area of myopectineal orifice was16.8±6.0cm2, whereas in females, it was8.0±5.3cm2. It can be made the reference of the height of body and pelvis to the size of mesh for Chinese people in tension-free hernioplasty. A prosthetic mesh measuring7.5×7.5cm is suitable.
