Objective:The aim of this study was to evaluate the association between mammographic features and the expressions of Ki-67in primary breast carcinomas and obtain a more accurate prediction of biological behavior of breast malignancies.Methods:A study of165preoperative mammograms in patients with breast carcinoma was performed. Mammographic features were evaluated according to BI-RADS. Mammographic findings were evaluated with an emphasis on mass shape, margin, density, calcification distribution, morphologic aspects, the presence of architectural distortion and Asymmetric density. Expression of Ki-67was determined by IHC staining. Correlations between the mammographic features of breast cancer and the expressions of Ki-67were evaluated.Results:Round mass shape and masses with a well-defined margin was more common in carcinomas with Ki-67low expression compared with carcinomas with Ki-67high expression (P=0.016, P=0.001, respectively). Lobular mass shape and masses with a microlobulated margin was more common in carcinomas with Ki-67high expression compared with carcinomas with Ki-67low expression (P=0.011, P=0.032, respectively). Ki-67expression was significantly higher in the group of patients with malignancy mass shape(lobular and irregular) than in those with benign mass shape(round and oval)(P=0.007), and was significantly higher in the group of patients with mass with malignancy margin(microlobulated, obscured and spiculated) than in those with mass with benign margin(well-defined)(P=0.001). Ki-67expression was significantly higher in the group of patients with higher mass density than in those with other mass density(equivalent, lower and fat equivalent)(P=0.004). Ki-67expression was significantly higher in the group of higher probability of malignancy calcifications than in those with higher probability of benign calcifications(P=0.012).Conclusion:Ki-67expression was significantly correlated with round mass shape, lobular mss shape, masses with a well-defined margin, masses with a microlobulated margin, malignancy mass shape, mass with malignancy margin, higher mass density and higher probability of malignancy calcifications. Mammographic diagnostic features may contribute to a more accurate prediction of biological behavior of breast malignancies.
