Background:Influenza is a viral infection that affects mainly the nose, throat, bronchi and, occasionally, lungs. Infection usually lasts for about a week, and is characterized by sudden onset of high fever, aching muscles, headache and severe malaise, non-productive cough, sore throat and rhinitis. There are three types of seasonal influenza-A, B and C. Worldwide, these annual epidemics result in about three to five million cases of severe illness, and about250000to500000deaths. In human history, there were four influenza pandemics. Among many subtypes of influenza A viruses, currently influenza A(H1N1) and A(H3N2) subtypes are circulating among humans for these years. In2009, there was a pandemic trend for Influenza A (H1N1) all over the world. By the end of2009, it have been reported that a total of over500thousand cases of Influenza A (H1N1) in208countries, of which more than11,500people were died from it. China is the high incidence rate country for Influenza, especially for the northern area. Jining city is in the west of Shandong Province in northern China, and the population was over8million. In jining city, influenza pandemic level is consistent with the whole country. Therefore, it is significant to learn the response capacity to prevent and control the influenza.Objectives:To learn the response capacity to cope with an influenza pandemic for the health system of Jining City, to analyze the existing conditions and vulnerable points in the influenza pandemic response for the health system of Jining City including the health administrative departments, centers for disease control and prevention, and medical institutions, in order to explore the basis for future events on pandemic preparedness, both political and academic ones.Methods:According to the unified request of project, the survey was conducted by the questionnaire and interview. The questionnaire of this survey was made by unified criteria list. The data was collected by Epidata software and analyzed by SPSS. Descriptive analysis and chi-square test were used to evaluate the response capacity to cope with an influenza pandemic in Jining city.Results:1. The response capacity to cope with the influenza pandemic for the public health administrative departmentJining city and the other12counties have established emergency early warning system of public health emergency and all the listed counties have organized the expert team for public health emergency, especially for the influenza pandemic, which is basiclly up to the requirement form state emergency command system.There are eleven counties have constructed the pandemic influenza pre-arranged planning, seven counties arrange the special medical institutes to care the cases of serious conditions.and most of them have carryed out the emergency exercise. Eight counties have constructed the pandemic influenza early warming system, and nine counties have made the training program for workers and the public, and most counties have the special institutes to take the pulic education.In2009, the health administrative departments of Jining City were able to abtain influenza surveillance information timely.12countis have developed the specialized agencies to supply, manage and depoly the emergency resources. Only four regions developed the expenditure budget of the daily work for public health emergencies. In the survey, it took388000/yuan per county for the public health emergency average in2009, but there were significant diffiences among counties.2. The response capacity to cope with the influenza pandemic for centers for disease prevention and contiol There are6centers that the professionals and technical rate is up to80%in Jining City, which is up to the Job design requirements. The coverage rate of network report is up to100%at or above the township level medical institutions in Jining City in2008and2009. There are two national influenza surveillance sentinel hospitals, two provincial influenza surveillance sentinel hospitals, and one national influenza surveillance network laboratory. At present, only five CDCs’s laboratory up to the P2standard, there is no P3level of the virus laboratory.In2009, there was no special fund for public health emergencies emergency in some of counties, which was not satisfied for the needed. It is suitable for the investagation for the equipments, and it is satisfied on the drugs and protective products. The problem is that the type and amount of the disinfectant is needed to be optimized.All the listed CDCs have constructed the pandemic influenza pre-arranged planning and the emergency exercise. Information communication skills was up to operational needs.3. The response capacity to cope with the influenza pandemic for medical institutionsAll the list hospitals have the the response capacity, and carried the education for their staff.13hospitals have been to the required standard. In the survey, the material related to the infecious disease was found, and specialized persons were responsible for infectious diseases and public health emergencies report. From2008to2009, the medical institutions are examined by the CDCs and other relevant departments for data omitting, but there are still some hospitals not to report all the required cases.There were few of hospitals that have obtained the annual expenses of public health in2008and2009, separately. In2009, only five hospitals got the fund for Influenza A (H1N1) prevention and control.In the surveyed hospitals, there was a good organizations institution for hospital infections system, and most of the nosocomial infection surveillance managers completed their work. The infection diseases departments in most hospitals were provided, and ventilation equipments, and the wards of respiratory disease were isolated from others. However, there was no negative pressure isolation room in all the hospital in Jining city. Conclusions:In this survey, the system for influenza pandemic have established and have played a great part in prevention and control the influenza pandemic in2009in Jining city. However, something must be done to improve the the response capacity by county level. Goods and meterials influenza releated should be supplyed, professional quality and detection level are urgent needed to improve.The response capacity of pandemic influenza can not meet the actual needs in Jining city. All levels of government should focous on the influenza prevention and control, and make more scientific and reasonable pre-arranged planning. CDCs should monitor the status of the disease further and improve the disposal capacity, and strengthen emergency exercise level. CDCs also have to develop training and education for the public. Medical and health institutions ought to improve the detection and treatment patient’s ability, and advoid hospital infection. Besides, they should strengthen sentinel hospital monitoring, in order to ensure the effective operation of the Influenza Surveillance Network.
