Objective:To examine if the OSAHS (Obstructive Sleep Apnea Hyperventilation Syndrome) patients suffer early renal impairment, observe the effects of n-CPAP therapy on patients, and preliminarily explore the mechanism of early renal impairment in OSAHS patients.Method:Assay the conventional index (Scr, BUN) and early sensitive index (Cystatin C, Blood β2-MG, Urine mAlb, Urine al-MG and Urine NAG) of kidney of30OSAHS patients and30healthy people, compare the differences between the two groups. Treat19moderate to severe OSAHS patients with n-CPAP therapy, and compare the changes of each index after the treatment. Analyze the parameter values(AHI. LSaO2、MSaO2、MAI) obtained by PSG, and the correlation between the index of renal function.Results:Experimental groups’Scr and BUN have no obvious differences with control groups (73.36±11.14.73.11±5.9,2.5±0.41、2.39±0.48)(P>0.05); Experimental groups’Cystatin C. Blood β2-MG、Urine mAlb. Urine al-MG and Urine NAG are significantly higher than the control groups (1.32±0.38.0.91±0.19,2.23±0.42.1.78±0.29,19±16.34.7.49±3.06,14.56±8.51.6.34±2.69,2.99±1.44、2.21±0.60)(P<0.01); After the treatment of n-CPAP, index in Scr. BUN and Blood β2-MG has no obvious differences with that before (73.77±13.52.72.65±11.56,2.51±0.45.2.46±0.42,2.33±0.42.2.25±0.43)(P>0.05); Index in Cystatin C、Urine mAlb. Urine al-MG and Urine NAG is significantly lower than that before the treatment of n-CPAP respectively (1.23±0.41.0.95±0.20,18.72±14.68.11.15±8.67,15.40±9.27.10.39±5.8,3.12±1.68.2.14±0.80)(P<0.01); The tranditional index of renal function, there is a negative correlation of Scr to MSaO2%(r=0.37,P<0.05), and there is a positive correlation of BUN to AHI (r=0.42,P<0.05); The early sensitive indices of kidney function Cystatin C, Blood β2-MG, Urine mAlb, Urine α1-MG and Urine NAG bear a positive correlation to AHI (r=0.600、0.591、0.564.0.524.0.586,P<0.01); Respectively, bear a negative correlation to LSaO2%(r=-0.556.-0.598、-0.600.-0.549.-0.515, P<0.01); And bear a negative correlation to MSaO2%(r=-0.545、-0.572、-0.557、-0.510、-0.493, p<0.01); Urine mAlb bear a position correlation to MAI, r=0.392, P<0.05.Conclusion:OSAHS patients do suffer early renal impairment, n-CPAP therapy has positive influences on the improvement of renal function of OSAHS patients, intermittent hypoxia may be the primary cause of OSAHS patients’early renal impairment.
