Acetabulum locates at the bottom of the middle part of the lateral hipbone, whichis constituted by ilium, ischium and pubis, showing hemispherical with deep concave,towards the front、down、outside, with the relevant section of the femoral head. As theoperation is always a tough problem of the orthopedic field, for many reasons such asthe deep position of acetabulum fracture, the complexity of anatomy process,difficulties of diaplasis fixation, many complications and so on. At present, at homeand abroad only a few scholars utilize manual anatomical measurement to doquantitative anatomical measurement of anterior column acetabular, and carry outquantitative anatomical study on the inner fixation with lag screw to the anterior andposterior column of the acetabulum, generally the study is carried on the few pelvisentities specimens, using cross-section method to carry through anatomicalmeasurement. Besides some other scholars adopted a computer-aid medical UniversalSurgical Integration System, which contains some tools with powerful function like3D segmentation and reconstruction tool, virtual dissection and measurement tool, todo quantitative anatomical study of acetabulum anterior column lag screw internalfixation. However, in actual surgical operation, due to the entry direction can be notmade accurate quantification, it generally uses C-arm to do X-ray navigationtechnology in the conditional hospital, but the surgery consumes3.8hours on average,doctors and patients may have already received too much radiation.This paper mainly carried out the study aiming at the above issues.①To directlyidentify CT scan data of the pelvic with medical image three-dimensionalreconstruction software, and reconstruct three-dimensional model of the pelvic, thenthree-dimensional model is imported to SolidWorks software, through optimization method do quantitative anatomical study to each cutting plane of the pelvis, thenmake synthetically judgments to select the screw entry point and screw entry direction.②The function research of aiming device at acetabulum anterior column, to design ahigh precision mechanical aiming device of setting nails in acetabulum anteriorcolumn by SolidWorks software, and derive the mathematical control model ofaiming device, at the same time do three-dimensional verification to slip angle of thedrill ringers by SolidWorks software, in addition, through to the designing,manufacturing and adjusting of the aimming device, also the aimming device usingprocess and aimming device using result, it is concluded that aimming device maybeexist some errors when it is used.③I n addition,fully considering the operationcondition, this paper combined with the man-machine engineering and mechanicalCAD/CAM technology, utilized three-dimensional feature modeling and virtualassembly technology of SolidWorks software, and developed the virtual machine ofsetting nail aiming device of accurate quantity and adjustable three-dimensional angle.The operators only need a short and simple training, input some datum of instrumentlike positioning spot, screw entry spots and three angles of screw entry direction inthe virtual prototype, namely the instrument is corresponding to the spatialrelationships of components for doing assembly constraint, and it will show the scaleposition of the instrument components in the interface, the operator can complete thephysical instrument adjustment based on virtual prototype.According to the virtual prototype system on high precision mechanical aimingdevice in acetabulum anterior column, the doctors can directly measure the data theyneeded by simulating the operative environment before operation,there is no need tofurther calculate and measuring detect, there is no need C-arm to do X-ray navigation,the adjusting process of all software and hardware can be completed in five minutes.Its significance is to reduce the difficulty and risk during operation, to improve theaccuracy, security and work efficiency of setting nail, which is prospective in clinicalapplication.
