Objective: To retrospectively study the assessment of early lactateclearance rate on prognosis of patients with respiratory failure.Methods: One hundred and seventeen patients with respiratory failureand elevated blood lactate were admitted into respiratory intensive care unit(RICU) ward. Arterial lactate and arterial blood gas were measured beforeand after treatment12h,24h,48h,72h, then12h lactate clearance rate werecalculated. The acute physiology and chronic health evaluationⅡ(APACHEⅡ)score was evaluated before and afer treatment12h in all the117patients. Patients were stratified by lactate normalization time, mortalityof each subgroup were compared.Patients were divided into the survivalgroup and nonsurvival group, clinical data were compare. The high lactateclearance rate group was defined as a lactate clearance rate≥10%, and thelow lactate clearance rate group with a lactate clearance rate <10%, mortalityand related data were compared. Results: Mortality of the lactate nomalized in less24hours group wassignificantly lower than that of the lactate not nomalized more than72hours(P<0.001). The12hour lactate clearance rate of survival group(43.6%±26.8%) was significantly higher than that of the death group(12.3%±39.1%)(P<0.01).Mortality of the high lactate clearance rate group(25.8%) was significantly lower than that of the low lactate clearance rategroup (71.4%)(P<0.01).Conclusion: early lactae clearance rate could be used as an reliablindicator for prognosis of patients with respiratory failure.
