| Background:Pulmonary embolism (PE) is a common disease with substantial morbidity andmortality. In recent years the researches have shown that the number of the patientswith PE is increasing year by year in China. When we make a correct diagnosis of PE,we must relate it to the severity of the patient’s assessment and provide the objective,and scientific basis for the Clinical Treatment decision-making, evaluation oftreatment, judgments of treatment outcomesObjective:To evaluate the severity of PE by quantitatively assessed the obstruction degreeof pulmonary artery by using pulmonary artery CT obstruction index. The obstructionseverity of pulmonary artery was assessed by the pulmonary artery CT obstructionindex combined with blood gas values, cardiac muscle symbol, thrombophilia testand D-dimer.Methods:Retrospective analysis was24cases of PE diagnosed by CT pulmonaryangiography (CTPA) in the hospital. The correlation between CTI with arterial Boodgas, cardiac enzymes and thrombophilia test were assessed by Spearman rankcorrelation analysis. The arterial blood gas values, thrombophilia test, D-dimer,Cardiac muscle symbol were respectively evaluated with different CTI cut offvalues(35%,45%,55%,65%)Result:1. The correlation between CTI and PaO2, and P(A-a)O2are significant and theCTI is an effective index to evaluate the severity of pulmonary embolism. PaO2and P(A-a)O2differed significantly at the55%and65%cutoff value respectively。2. The correlationes Between CTI and Thrombophilia test, D-dimer are presented by the changes of the APTT, INR, D-dimer, PT. For the patients with PEthe APTT is differed more significantly than the PT, INR and D-Dimer by using the55%and65%cutoff values. PT is differed more significantly than the other threeindicators using the55%and65%cutoff values in Patients with PE。3. The correlations Between CTI and Cardiac muscle symbol are significantthrough the changes of ALT, AST and LDH. They are also the effective indexes toevaluate the severity of pulmonary embolism. ALT and AST are differed moresignificantly than LDH at the35%cutoff. The three indicators are differedsignificantly between at the45%,55%and65%cutoff in Patients with PE.Conclusions:In patient with PE, The correlations were existed between blood gas value,Cardiac muscle symbol, Thrombophilia test, D-dimer and pulmonary artery CTobstruction index. These Biochemical indicators can be used to assess the clinicalseverity of PE. |
