Objectives: We designed this study to assessed Ki-67protein expression inyoung patients with gastric cancer.Methods: one hundred and twenty five cases of gastric cancer in patients40-years-old and younger, occurring in the period January2001to January2006arereviewed. The pathological diagnosis of these patients was confirmed by surgery andendoscopy. Gastric cancer tissue from125patients was immunohistostaining forKi-67protein. Ki-67Label index was defined as the percentage of tumor cellspositive for Ki-67protein. The relationship between Ki-67LI and clinicopathologicparameters, patient overall survival were sought.Results: Ki-67LI was correlated with gender, Histologic type, Laurenclassification, T stage, Lymph node metastasis, Distant metastasis(P<0.001). Inyoung gastric cancer, the High LI groups were found to have a significantly worsemedian survival time than the low LI groups(P<0.001). Multivariate analysisrevealed that High Ki-67LI, poorly differentiated histology, an advanced T stage,diffuse type, lymph node metastasis, and distant metastasis are independentprognostic factors in young patients with gastric cancer. The gender stratificationanalysis showed that Ki-67LI is an independent prognostic factor for survival inyoung female patients with gastric cancer (HR=2.545,95%CI (1.198~5.405), P=0.015).Conclusions: Ki-67LI is an effective tool for predicting survival in younggastric cancer, especially is an independent prognostic factor in young female patientswith gastric cancer.
