Background With the quicken pace of global aging,the incidence of cognitivedysfunction increases in the elderly population gradually.But with the in-depthresearch,scholars have come to realize the concept of dementia has laggedapparently.So mild cognitive impairment (MCI) were raised which is an early stage ofdementia.The risk of dementia is10times higher than the normal elderlypeople.Therefore, early detection and intervention of MCI become so important.A largenumber of studies show that cerebral white matter lesions(WML) and cognitivedysfunction are closely related, even WML is an independent risk factor.This studyexplores the influence on cognitive of different levels and parts of WML in senile malepatients and discuss the risk factors of cognitive impairment to provide a theoreticalbasis for later clinical prevention.Part1The prevalence of WMLand cognitive situation in senile male patientsObjective Screening out the patients of cognitive dysfunction with WML.Methods Selecting179senile male patients during2010March-2011March inthe Department of Neurology. Collecting the clinical information and psychologicalassessment scales were performed.Results There are114WML patients(63.68%) of all the participations. Amongthem,44MCI patients,4AD patients,20mixed dementia(MD) patients.Conclusion There is a high prevalence rate of WML in the senile male patients inthe army, especially the subset above80years old.Part2Characteristics of cognitive function of senile male patients with whitematter lesionsObjective To explore the characteristics of cognitive dysfunction in senile patientswith white matter lesions.Methods The MoCAscales were performed to64patients with different extent of WML, and the relationship between the cognitive function and the extent of WML wasevaluated.Results The WML can cause the cognitive function decline.1. PVL MoCAscoresin moderate-severe group showed significant difference. Cognitive declines in thedelayed memory, orientation,attention(P <0.05);2.DWML The function of namingshowed significant difference between the moderate-severe WML and the controls (P <0.05).Conclusion WML may cause impairment of cognitive function, and PVL may bemore serious.Part3The risk factors of cognitive dysfunction in senile male patients withwhite matter lesionsObjective To explore the risk factors of cognitive dysfunction in senile malepatients with WML.Methods64patients with WML were classified into cognitive impairment andnormal cognitive group. Logistic analysis was made on the demographic date andvascular risk factors.Results Cognitive impairment presented in31patients.Logistic regression analysisidentified the presence of cognitive impairment is significantly associated witheducation(OR0.867,95%CI0.789-0.953)、smoking(OR5.151,95%CI1.646-16.123)、lacunar infarction(OR3.766,95%CI1.080-13.130).Conclusion Our study confirms that the cognitive impairment in senile male WMLpatients and is significantly associated with education、smoking、lacunar infarction.
