Objective: Ovarian cancer is women’s common malignant tumor, is also the most pathological type classification malignant tumors. Due to the difficulty to find in an early stage, and spreading or metastasizing easily in the abdomen and pelvic cavity, the high incidence and high mortality rate in women malignant tumor take the first place. Insidious onset of ovarian cancer, lack of specific symptoms and effective diagnostic and therapeutic tools result in advanced stage and poor prognosis when diagnosed. The treatment of ovarian cancer at present is combined chemotherapy on the basis of cytoreductive surgery together with platinum-based chemotherapy (including systemic and intraperitoneal chemotherapy) to improve treatment efficiency and patient survival rate. But chemotherapy can easily produce resistance, which causes poor efficacy. The purpose of this study is to explore with ovarian cancer affects the treatment outcome and the related factors.Methods:Carrying out a retrospective study of123patients with epithelial oyarian cancer treated in Nanjing Drum Tower Hospital from January2004to December2009, analyze survival time by Kaplan-Meier, test survival rate differences by log-rank and take multivariate analysis by Cox proportional hazards model. So P <0.05was considered statistically significant.Results:The5-year survival rate of stage â… , â…¡ epithelial ovarian cancer patients was significantly higher than those of stage â…¢, and â…£ patients (P<0.05); Differentiation in high-for five years, low patients survival rates were39.7%and11.5%respectively, poorly differentiated patients5-year survival rate is far lower than the well-differentiated surial difference have obvious statistical significance (P <0.05); the5-year survival rate of patients without residual tumor by naked eye after surgery was50.6%, the5-year survival rate with residual tumor≤2cm was27.0%. Both of their5-year survival rates were6.7%(P <0.05) higher than the residual tumor>2cm; the5-year survival rate of patients of40-60years old was41.3%, while≥60years old was26.7%. The5-year survival rate of patients with ovarian cancer without lymph node metastasis was53.5%, patients with ovarian cancer with lymph node metastasis was5.6%. By the test, the difference was statistically significant,(P <0.05) Postoperative chemotherapy>6times, the5-year survival rate was33.8, and chemotherapy≤6, the5-year survival rate was19.1%, the difference of which was statistically significant (P <0.05).Conclusion: The clinical stage ovarian cancer, differentiation degree, lymph node metastasis, surgery focal size, postoperative chemotherapy residual are ovarian cancer prognosis factors, and differentiation degree and operation residual focal size, postoperative chemotherapy treatment is ovarian cancer independent prognostic factors.By analyzing the standard surgery of ovarian cancer in the early stage and initial surgery in patients with advanced stage to remove the tumor lesions with the utmost possible effort, Supplementary in sufficient, effective and standard chemotherapy can improve the survival rate and quality of life of patients with ovarian cancer after surgery, residual lesions <2cm. |
