Object ive:Retrospective study of surgical cases Transoral Atlantoaxial Reduction Plate Internal Fixation(TARP)combined traditional Chinese medicine methods for treatment of Basilar invagination with syringomyelia, to observe the clinical efficacy and the change of syringomyelia size.Methods:From March2005to July2011, the TARP system was treated75cases of patients with basilar invagination, including35patients with syringomyelia. we have followed and analyzed the32patients with basilar invagination and syringomyelia retrospectively to follow-up the change of symptoms,to calculate the changes of cervico-medullary angle, to statistic the complications of preoperation and postoperation;we were compared preoperative and postoperative changes of JOA and ASIA scores, calculating the rate of improvement, quantifying syringomyelia size utilized Bit software company Angel pigment analysis and stereological methods, observing the preoperative and postoperative changes of syringomelia and bone fusion.Results:The32patients followed up course of diseases were6months to372months, an average of81.3±70.9months;clinical symptoms were all sorts, the preoperative MRI sagittal average cervico-medullary angle wasl23.3°±6.4°, the postoperative cervico-medullary average was recoveryed to161.3°±5.1°. The32cases were non-postoperative complications, the JOA score from preoperative (3to16points, average9.3±4.3points) improved to postoperative (10to16, average13.8±1.8points), with an average improvement rate58.1%, ASIA score preoperative average C-class to improve to the postoperative average of grade D; after one week, after three months to review the MRI, cervico-medullary angle immediately back to normal intrapteration, syringomyelia size had reduced after one week.3months afterwards the syringomyelia size had significantly reduced, preoperative syringomyelia size average2.49±3.88cm3, postoperative syringomyelia size average2.49±3.88cm3after one week, postoperative syringomyelia size average0.42±0.52cm3after3months, the internal fixation without displacement, fusion in good condition after postoperativel week,postoperative3months.Conclusion:Transoral Atlantoaxial Reduction Plate Internal Fixation combined traditional Chinese medicine methods for the treatment of invagination with syringomyelia could get good clinical effect, which could obtain axis odontoid anatomical reduction and signicantly reduce syringomelia size and markly improve patients clinical symptoms, but long-term efficacy needs further obervation.
