| Purpose:To study advantages and disadvantages of different surgical approaches for candylar process fracture.in order to choose the best surgical approach in clinical treatment.Material and Methods:Patients with candylar process fracture retrieved from the department of stomatology in the First Affiliated Hospital of the college of Medicine in Zhejiang University between March2009and October2011; collect information such as gender,age,cause,fracture position,whether there were other fractures.surgical approach,internal fixation,and whether there was nerve injury and occluding relation.Results:There were78patients with86fractures of condylar process, joint capsule fractures in25patients, and at neck fractures of condyle38cases, low fracture9cases,sagittal fracture14cases. A total of75cases of condyle fractures were done surgical operation including64cases preauricular approach, temporoauricular approach7cases,retromandibular approach3cases,submandibular approach lcase;in75postoperative cases, there were10cases of facial nerve injury,including5cases in temporal branche and3cases in zygomatic branche,l case in marginal madibular branch,lease in buccal branche,which got right in3months after operation.6cases of occlusal were bad,but return normal after2weeks intermaxillary traction.75cases of the joint function were basic normal, wound healing was good, the scar was not obvious;limite of open the month in6month reached normal.Conclusions:The temporoauricular approach is appropriate for many types of surgical treatment of fractures including capsule fracture, neck fracture and part of low fracture,which has the advantage of incision concealment, postoperative function recovery effect, facial nerve damage and other less complications.So it is the ideal of approach. |
