The Association Study Of Comfortable Level Of The Upper Abdomen Use Following Pedicled Skin Flap Holder

Objective：In this case we use different fixed modes to fix patients after ilium inguenflap operation,then compare the comfort level of patients with differentmethods，to explore a better way to fix limbs.In order to elevate the extremity，promote blood supply of the flap,reduce pedicle time and increase the comfortof patients.Method：Select60cases which do upper limb iliac groin following pedicled skinflap transplant operation at department of hand and foot surgery，the firsthospital of jilin university during june2010to December2011.Randomlydivided them into the experimental group and the control group.We let patientsin experimental group use self-made following pedicled skin flap holder tofixed limb wheel control group patients put traditional phantom cotton pillowmat under limb to fix limb. Through the observation of the situation of the flapand physical pain of diseased side shoulder and limb，sleep quality，pedicledivision time overall comfort level， we can make the conclusion withcomparing the two kinds of method to the influence of the patients.Result：The fixed way of the experimental group is better than that of the controlgroup, specifically As Follows：1、We have30patients in both groups，observe the pedicle of the flap afterthe operation，experimental group1563person time and Flap position right1490person time，wheel control group1546person time and Flap position right1187person time.Compare the wo groups (P <0.01)，statisticallysignificant.2、Measured with the pain index of patients’ shoulders and upper limbs at12h，24h，48h，72h after the operation，Positive index is≥4，compare the painindex of patients’ shoulders of experimental group and control group，P＜0.01at12h，24h，72h， when P＜0.05at48h，statistically significant.Compare thepain index of patients’ limbs of experimental group and control group，P＜0.05at12h，P＞0.05at24h，48h，72h，no statistical significance.3、Through the GCQ comfortable condition rating scale， two groups ofcomfort contrast P <0.01，statistically significant.4、Pedicle time，19-23days of the experimental group，20.70days onaverage；19-24days of the control group，21.57days on average， P <0.05，statistically significant.5、Sleep time Surgery that night，the experimental group2-11h，6.35h onaverage；The control group0-11h，4.87h on average，P <0.05，statisticallysignificant.Conclusion1、Upper abdomen following pedicled skin flap holder provide accurateAngle and height for upper limbs iliac groin following pedicled skin flaptransplant limb fixed.2、Upper abdomen following pedicled skin flap holder is able to help propup the limb，make the limb comfortable.3、The fixing band of upper abdomen following pedicled skin flap holdercan fix the limbs well when the patients sleep,make the limb stable，to ensurepatients sleep，to prevent the pedicle of the flap reverse，fold or avulsion whenpatients sleeping. 4、Using upper abdomen use following pedicled skin flap holder，canreduce pedicle time，and prevent or reduce complications occured,increasepatient’s comfort level.
