Objective:To summarize common intraoperative and postoperative complications following mandibular distraction osteogenesis(DO) in the clinical application, in order to find effective methods for prevention and management.Methods:Retrospective analysis of30cases underwent DO in our hospital from2007to2011,maxillary benign tumor underwent mandibular defect repair in12cases, used a total of12distractors, unilateral temporomandibular joint ankylosis in6cases and bilateral temporomandibular joint ankylosis in12cases, used a total of56distractors.Results:30cases of DO for treatment of mandible defects caused by benign tumors and temporomandibular joint ankylosis completed distraction successfully in6-8months and remove the distractors, used a total of68distractors.Facial scar occurred in every cases,1case of mandible defect caused by benign tumor became open bite,2cases of temporomandibular joint ankylosis had temporary numbness of the lower lip in which recovered in three months,3cases had facial nerve injury symptoms:distortion of commissure which recovered in a few days, titanium plate rejection with infection occurred in2cases, damage of teeth and tooth germ occurred in2cases, besides, there is each1case which occurred infection, poor osteogenesis, pathological fracture, distractor fracture or temporal pain.Conclusions:The keys to reducing the DO intraoperative and postoperative complications is to be familiar with the anatomy of mandible, design reasonable mode of operation and location of distractor, predict postoperative occlusion and produce personal distractor, do reasonable postoperative distraction and recheck regularly to see growth condition of mandible and avoid fracture of distractor. It is necessary to preoperative preparation and postoperative care.
