Background: With the change of lifestyle and environmental factors, theincidence of hypertension is more and more high. In the long-term hypertensionand poor control of blood pressure, the left ventricular hypertrophy occurs at avery high rate.Long term left ventricular hypertrophy can lead to coronary heartdisease, heart failure, arrhythmias and even sudden cardiac death, cansignificantly increase the incidence of cardiovascular events and mortality.More and more studies show that left ventricular hypertrophy is an independentrisk factor of cardiovascular event. Therefore, reducing the degree of leftventricular hypertrophy and cardiac remodeling has aroused wide attention. Atpresent, our goals of treatment in patients with hypertension not only aneffective and smooth control of blood pressure, and slow down or reverse leftventricular hypertrophy, and it is one of the main objectives. A large number ofstudies show that blood pressure on left ventricular hypertrophy and long-termpoor control at a high level is closely related. But in the clinical work, we oftenfind some hypertensive patients although have a long-term blood pressureunder the satisfactory controlled, but left ventricular weight is still on theincrease. So we think that the circulating and local neuroendocrine hormonemay be played a more important role in left ventricular hypertrophy occurs anddevelopment. APN is secreted from fatty tissue with similar structure of thecollagen protein. In patients who has obesity, insulin resistance, type2diabetes,hypertension or coronary heart disease, plasma adiponectin levels weresignificantly lower than normal. The CT-1from the embryo of the mouse heartstem cells cloned is a novel cytokine, can induce cardiocytes hypertrophy, which is one of IL-6family members. Studies have shown that CT-1exists in a variety of tissues, highest expression in myocardial tissue, plays an importantrole not only in heart development and promotion of cardiachypertrophy.Research has confirmed that adiponectin in hypertension patientsis decline and cardiotrophin-1is rise. And the correlation research ofadiponectin and myocardial nutrients-1in hypertension patients is rare. Objective: Investigate the relationship of serum adiponectin andcardiotrophin-1changes in hypertension patients and its possible formationmechanism and risk factors.Method: A total of94cases of the object of study, from2010Decemberto2011November in the First Hospital of Jilin University cadre examinationand hospitalized patients.64cases of hypertension patients, between them thereare31left ventricular hypertrophy cases and33non left ventricularhypertrophy cases. All cases were consistent with the"Chinese hypertensionprevention guide"(revised edition in2005) to determine the diagnostic criteriaof hypertension, LVH diagnostic criteria for left ventricular mass index(LVMI)>125g/m2(male) or>120g/m2(female). Select age, gender, weight matched30patients as a healthy control. Patients were excluded fromsecondary hypertension and unmotivated, cerebral, renal dysfunction anddiabetes mellitus. All subjects were measured for height, weight, blood pressure, fasting blood glucose, blood lipid and biochemical examination, determinationof ELISA two groups of serum adiponectin and cardiotrophin1(CT-1)concentration. Ultrasonic heartbeat graph determination in patients with leftventricular end diastolic diameter (LVEDD), end diastolic left ventricularposterior wall thickness (LVPWT), end diastolic interventricular septalthickness (IVST). According to the formula of left ventricular weight (LVM)and left ventricular mass index (LVMI). For statistical analysis, P <0.05withstatistical significance.Results:①Adiponectin level in the group of hypertensive with left ventricular hypertrophy and hypertension without left ventricular hypertrophygroup were significantly lower than those of healthy controls (P <0.01),between them, the adiponectin level in the group of hypertensive patients withleft ventricular hypertrophy is lower than another, it has statistical significance(P <0.01).②CT-1level in the group of hypertensive with left ventricularhypertrophy and hypertension without left ventricular hypertrophy group weresignificantly higher than those of healthy controls (P <0.01), between them, the CT-1level in the group of hypertensive patients with left ventricularhypertrophy is higher than another, it has statistical significance (P <0.01).Conclusions:1.Serum APN level of hypertension in patients isreduced, Serum CT-1level of hypertension in patients is rised, while LVHin combination, serum APN levels is significantly decreased. Serum CT-1levelof hypertension in patients is significantly rised,2.Serum adiponectin level in hypertension patients and leftventricular mass index (LVMI) has positive correlation, CT-1level and LVMIhave positive correlation, APN and CT-1in hypertension patients with leftventricular hypertrophy are closely related, they play important roles in thedevelopment of hypertensive with left ventricular hypertrophy.
