ObjectiveInvestigate the clinical and pathological data characteristics of patients with rectal cancer and tumor cell shedding in the correlation of intraperitoneal free cancer cells in order for intraoperative peritoneal washing (or chemotherapy) provide a theoretical basis.MethodsRandomly selected to the First Affiliated Hospital of Zhengzhou University, ano-rectal surgery with complete clinical data of71patients with rectal cancer,69cases of patients undergoing resection of the tumor to close the abdominal cavity with500ml normal saline flush abdominal, two cases of direct extraction of peritoneal effusion, no-peritoneal washing, and immediately sent to pathological examination by light microscopy to detect free cancer cells collected rinse fluid or peritoneal effusions, record results and statistics, the establishment of a logistic regression model, univariate and multivariate analysis, statistical analysis of related factors on rectal intraperitoneal free cancer cells.ResultsThe analysis showed that(1) Tumor size>5cm(P=0.012, OR=1.729), ulcerative carcinoma (P=0.049, OR=2.500), tumor breakthrough serou (P=0.011, OR=7.538), Dukes stage (B stage:P=0.003, OR=3.401; C stage:P=0.000, OR=6.600) and lymph node metastasis (P=0.000, OR=2.618) were significantly associated with FCC in the abdominal cavity.(2) Dukes stage was risk factors for detection positive of FCC after surgery (B stage:P=0.000, OR=4.120; C stage:P=0.000, OR=7.231) Conclusion(1) Resection of rectal cancer surgery does not increase cancer cell shedding to strictly follow tumor surgery tumor-free principle.(2) Intraoperative detection of exfoliated cells to help early detection of colorectal cancer micrometastases in the peritoneal lavage fluid.(3) Specially to the patients of Dukes stage C, tumor diameter5cm, lymph node metastasis, It is required to intraoperative peritoneal soak and peritoneal lavage with normal saline(or distilled water)containing chemotherapeutic agents.
