Objective:To explore the microsurgery treatment and follow-up of anterior communicating artery aneurysms. Methods41patients with anterior communicating artery aneurysms From January2007to august2011underwent microsurgery.The clinical, radiographic and microsurgery treatments were analyzed retrospectively. Results Preoperative DSA or CTA could showed the projections of anterior communicating artery aneurysms clearly, as well as the A1segment dominant signs character. All the aneurysms were clipped successfully.6cases with Intraoperative rupture of aneurysm had been controlled effectively.41cases were discharged and assessed with GOS scores28cases of good recovery,9cases of moderate disablity,1case of severe disablity,2cases of vegetative state,1case of death.37cases Follow-up3-52months without rupture of aneurysms. Conclusion The microsurgery of anterior communicating artery aneurysm is exact and effective. Acute surgery can reduce the chances of aneurysm rebleeding after subarachnoid hemorrhage,but involve in high operational risk and chance of craniectomy. Selecting the appropriate patients with cerebral angiography and Hunt-Hess grade is necessary. It is important that Preoperative understand the projections of aneurysms, temporary occlusion of artery and proficiency in Microsurgical techniques to reduce aneurysm rupture and surgical injury.
