Purpose To investigate the value of sequential segmental analysis when diagnosing heart malformation of congenital heart disease by MSCT, through the application of this analysis method in analyzing 64-slice spiral CT images of this type of congenital heart disease.Methods The materials of 98 cases examined by 64-slice spiral CT and echocardiography, which confirmed with heart malformations of congenital heart disease by operation, were analysised. Among these cases, male:63, female:35, age:1 month-47 years old, mean age:10.07±9.78 years old; All cases were underwent plain and enhancement scanning by GE lightspeed VCT 64-slice spiral CT, technology of the Bolus tracking in enhancement scanning was adopted. The images obtained were transmitted to ADW 4.4 post-processing workstations, using reconstruction methods just like multi-planar reconstruction (MPR), maximum intensity projection (MIP) and volume rendering (VR) to analyze. We observed the position, morphological changes and merged malformations of Cardiovascular, using sequential segmental analysis as a diagnostic method, listed morphological deformities of the corresponding segments, and then carried out statistics, finally, the results of 64-slice SCT were comprehensively compared with the corresponding echocardiography and surgical results.Results (1) 64-slice spiral CT demonstrated morphological deformities of heart malformation among congenital heart disease:the deformities of atrial section were 66,64-slice CT found 44, the definite diagnostic rate was 66.7%; the deformities of ventricular section were 179,64-slice CT found 178, the definite diagnostic rate was 99.4%; the deformities of atrioventricular section were 18,64-slice CT found 18, the definite diagnostic rate was 100%; the deformities of aorta section were 101,64-slice CT found 94, the definite diagnostic rate was 93.1%; the deformities of ventricular arterial connection section were 148,64-slice CT found 133, the definite diagnostic rate was 89.9%; cardiac malpositions were 13,64-slice CT found 13, the definite diagnostic rate was 100%. (2)There was no significant difference between 64-slice spiral CT and color Doppler ultrasound echocardiography which used to diagnose abnormalities in the aspects of the atrial section, ventricular section, atrioventricular section, ventricular arterial connection section and cardiac malposition; there was significant difference between 64-slice spiral CT and echocardiography which used to diagnose abnormalities in the aspects of the aorta section. The accuracy of CT diagnosis in abnormalities of the aorta section was significantly superior to ultrasound. (3) There was no significant difference, when the measurement of 64-slice spiral CT compared with the surgical findings and ultrasound diagnosis on atrial septal defect(ASD) and ventricular septal defect(VSD).Conclusion (1) The scanning speed of 64-slice spiral CT is very fast; 64-slice spiral CT which has high temporal resolution, spatial resolution and density resolution, achieves isotropic.64-slice CT can display all kinds of abnormal cardiovascular structures in heart malformations of congenital heart disease comprehensively, accurately and intuitively, with the help of images reconstructed by a variety of post-processing techniques, so it is an important non-invasive examining techniques of congenital heart disease.(2) The detection rate of 64-slice spiral CT angiography has no obvious advantages compared with that of echocardiography in atrial, ventricular section, atrioventricular connections, ventricular arterial connection section and cardiac malposition of heart malformations, but about abnormalities of the aorta section,64-slice CT is superior to echocardiography; However, CT can not show valvular changes; The complement, combination diagnosis of 64-slice spiral CT and echocardiography can raise accuracy in diagnosing cardiovascular structural abnormalities of congenital heart disease, and find out the hemodynamic changes. (3) Cardiac examinations of 64-slice spiral CT with the help by varieties of post-processing techniques, in coordinate with the sequential segmentation analysis method, could find out kinds of abnormal morphological changes of congenital heart disease comprehensively and correctly, and provide affluent and intuitive imaging information for the establishment of clinical protocols.
