AIM To investicell gate effect of expression of hypoxia inducible factor-1 alpha(HiF-1α)and T-lymphoma invasion and metastasis factorl (Tiam1) on invasion and lymph node metastasis in esophageal carcinoma and its prognostic significance.METHODS The protein expression of HiF-1αand Tiam1 was detected by immunohistochemistry in 33 esophageal carcinoma specimens,15 normal esophageal mucosal specimens, and the relationship between the protein expression and T stage、N stage、clinical TNM stage、tumor length、histological grades was analyzed, and prognostic factor in esophageal carcinoma treated by radiotherapy was evaluated.RESULTS The positive rates of HiF-la and Tiaml protein expression in esophageal carcinoma were 66.7% and 72.7% respectively, and both 6.7% in normal esophageal mucosal specimens (P<0.01); Level of expression of HiF-la in esophageal carcinoma was positively correlated with N stage(P<0.05), Level of expression of Tiam1 was positively correlated with clinical TNM stage and N stage(P<0.05),The expressions of HiF-1αand Tiam1 all did not correlate with age、gender、tumor length histological grades and T stage (P>0.05); The complete effective rate and mean survival period after radiotherapy were 9.1% and 9 months respectively in Hif-la-positive group in esophageal carcinoma, while in Hif-la-negative group 63.6% and 25 months respectively (P<0.05),The 1-,2-,3-year survival rates after radiotherapy were 40.9%、13.6% and 6.8% respectively in HiF-1α-positive group, while in Hif-la-negative group 90.9%、54.5% and 36.4% respectively. Log-rank test suggested that the difference was significant (P=0.007); Multivariate Cox regression analysis revealed that Over expression of HiF-1αwas independent prognostic factors; There were positive correlations between HiF-1αand Tiam1 protein in esophageal carcinoma (r=0.433 P=0.012)CONCLUSION High expression of HiF-1αand Tiam1 protein have a promoting role in lymph node metastasis in esophageal carcinoma; Over expression of HiF-1αhad the resistance effect on esophageal carcinoma treated by radiotherapy, and was considered as independent prognostic factor after radiotherapy.
