Objective To compare the effects of high right atrial (HRA), distal coronary sinus (CSd) and coronary sinus ostium (CSO) pacing on atrial electrophysiological parameters.Methods Patients underwent a intracardiac electrophysiological study were enrolled in this study from November2011to February2012. Electrode were placed at high right atrial,CSO and CSd to record intracardiac electrogram,and surface electrocardiogragm (ECG) were record at the same time. The fellowing parameter were measured during sinus and pacing model:P wave duration, atrial activation duration,time different between right and left atrial activation and atrioventricular conduction delay.Results16patients were enrolled in this study. Mean ages were36.5±9.4years.Results of electrophysiological study were as follow: Comparing with HRA (140.3±10.3ms) and CSd pacing(139.3±10.8ms) P wave duration is shortened during CSO pacing (87.2±9.7ms).The atrial activation and time different between right and left atrial were prolonged during HRA pacing (126.7±14.6ms vs90.7±18.2ms) and CSd pacing (135.3±10.2ms vs75.0±13.0ms) compared with CSO pacing (100.0±12.6ms vs33.0±18.6ms),and the difference was statistic significant.CSO pacing have shortened AV delay (138.3±5.0ms) compared with HRA (199.9±30.4ms) and CSd pacing (213.7±61.0ms).Conclusion Atrial activation duration are markedly shortened and the dispersion of atrial activation are reduced during CSO acing,indicate that CSO pacing is in favor of biatrial synchronization,and may have beneficial effect on the prevention and treatment of paroxysmal atrial fibrillation.
