PartⅠAnalysis of the Risk factors for small-artery occlusion strokeObjective To investigate the differences of clinical and biochemical risk factors of small-artery occlusion stroke (SAO) and its subtypes.Methods171stroke patients with SAO,120with large-artery atherosclerosis(LAA)from January2009to November2011were enroued prospectively.All patients received cranial magnetic resonance imaging at the first week.Homocysteine levels and other biochemical parameters were measured on the next day of addmission.All data were selected from Nanjing Stroke Registry Program.Results Age, hypertension and were significantly higher in SAO than those in the LAA group (all.P<0.05). Through logistic regression analysis,age (OR=1.047, CI=1.00~1.09, p=0.043) and history of hypertension (OR=2.632, CI:1.08~6.41, p=0.033) were risk factors for SAO.Among a total171SAO patients87(50.88%) patients were in IIA group,84patients(49.12%) were in ILA group. Age(OR=0.041,95%CI:1.00~1.08;P=0.030), history of hypertension (OR=1.383,95%CI1.56~11.14;P=0.00) were risk factors for ILA. Hyperlipidemia (OR=0.136, CI=0.50~0.369,P=0.000) was a risk factor for ILI..Conclusion SAO has a different risk factor profile from the typical atherosclerotic profile found in LAA, with hypertension being important. There are differences in the risk factor profile between the SAO subtypes; age and hypertension are risk factors for ILA while hyperlipidemia is a risk factor for ILI. PartⅡAssociation of matrix metalloproteinase-2and matrix metalloproteinase-9single nucleotide polymorphisms genes with risk of smll artery occlusion stroke[Abstract] Objective To analyze the matrix metalloproteinase-2(MMP-2) and matrix metalloproteinase-9(MMP-9) single nucleotide polymorphisms associated with smll artery occlusion (SAO), investigate the mechanism of SAO.Methods198stroke patients with SAO from January2010to November2011in the Nanjing Stroke Registry Program were enroued prospectively, and chose contemporaneity control group (n=100) matched forage, gender. The genotype of MMP-2-1306T/C and MMP-9-1562C/T were analyzed by the TaqMan SNP Genotyping Assay. Results.The genotype frequencises of MMP-2-1306T/C and MMP-9-1562C/T in SAO were similar to those in control subjects (P>0.05) Further Subgroup analyzed, the genotype frequencises of CC genotypes in MMP-2-1306T/C polymorphisms was significantly increased in ILA when compared with cntrols (80.0%versus66%, P=0.024), and the genotype frequencises of C allele in MMP-2-1306T/C was also significantly increased in ILA when compared with cntrols (89.0%versus81%, P=0.022). The genotype of MMP-9-1562C/T in ILA was similar to those in control subjects; the genotype frequencises of MMP-2-1306T/C and MMP-9-1562C/T in ILI were similar those in control subjects (P>0.05. Further according to age, hypertension and MMP-9polymorphisms stratification studied,>65years old (OR=5.67,95%CI1.91-16.80) and the history of hypertension (OR=5.04,95%CI1.787-14.25) were marked increased genotype of MMP-2CC genotype people the risk for ILA; MMP-9-1562C/T genotype and MMP-2-1306T/C genotype did not interact, MMP-9-1562C/T genotype were not increased genotype of MMP-2CC genotype people the risk for ILA.Conclusion ILA and ILI are two different types SAO. MMP-2-1306T/C polymorphisms are associated with ILA in Chinese people. Age and the history of hypertension are marked increased genotype of MMP-2CC genotype people the risk for ILA.
