Objective:Detection the activity of atrial fibrillation (AF) in patients with serumaldosterone, C reactive protein and the brain natriuretic peptide (BNP) toexplore aldosterone, C reactive protein, brain natriuretic peptide on cardiacstructure and the incidence and maintenance of atrial fibrillation, to provide atheoretical basis for the treatment and prevention of atrial fibrillation.Methods:Select patients with atrial fibrillation of70cases from January2010toMarch2012, the Sino-Japanese Friendship Hospital of Jilin UniversityDepartment of Cardiology and Geriatrics met the study criteria. Including40cases of paroxysmal atrial fibrillation were22males and18females, age (65~88) years old, the average (76.03±6.52) years old; Patients with persistent AF(n=30), including17males,13females, age (64to85) years old, theaverage (74.1±5.75) years old;20cases selected the same period inhospitalized patients with sinus rhythm control group were10males and10females, age (65to81) years old, the average (73.4±4.15) years old. Threegroups of subjects of gender, age and underlying diseases (coronary heartdisease, diabetes, high blood pressure) situation was no statistically significant(P>0.05), were comparable. Three groups of subjects before in medication thesober fasting state extract elbow blood5ml, then use centrifuge separate theserum specimens,using liquid equilibrium competition radioimmunoassaydetermination of serum aldosterone levels, using immune turbidimetric methoddetermination of serum CRP, using the enzyme linked immunosorbent assay(ELISA) determination of serum BNP levels. Three groups of study left atrial diameter measured through the heart of color Doppler ultrasoundexamination.Obtained data were statistically analyzed using spss13.0statisticalpackage, serum aldosterone, CRP, BNP and left atrial diameter of threegroups of subjects were compared;left atrial diameter as the dependent variable,serum aldosterone, CRP and BNP levels as independent variables, multipleregression analysis.Results:(1) paroxysmal AF and persistent AF levels of serum aldosterone, CRPand BNP level were significantly higher than the sinus rhythm group, astatistically significant difference (P<0.01); persistent atrial fibrillation patientsthe serum above cytokines were significantly higher than patients withparoxysmal AF, the difference was also statistically significant (P<0.01).(2) paroxysmal AF and persistent AF patients with left atrial diametermeasurements were significantly higher than the sinus rhythm group, astatistically significant difference (P<0.01); Determination of persistent atrialfibrillation patients with significantly higher left atrial diameter values thanpatients with paroxysmal atrial fibrillation group, the difference wasstatistically significant (P<0.01)(3) paroxysmal AF and persistent AF patients with left atrial diameter asthe dependent variable, serum aldosterone, CRP and BNP levels asindependent variables, multiple regression analysis results showed thataldosterone, CRP and BNP increase in left atrial diameter expansion of thelinear relationship between the regression coefficients were statisticallysignificant (P<0.05)..Conclusion:Patients with atrial fibrillation serum aldosterone,C-reactive protein, andBNP levels were significantly higher, indicating that these indicators haveplayed an important role in the incidence of atrial fibrillation. These indicatorsin the patients with persistent atrial fibrillation increased more, with the longer duration of AF, these indicators increased more obvious, indicating thatpatients with atrial fibrillation may through the induction of aldosterone, Creactive protein and BNP expression promote the development andmaintenance of atrial fibrillation. Atrial fibrillation in patients with left atrialdiameter increased significantly, indicating that atrial remodeling is closelyrelated to atrial fibrillation, and atrial structural changes promote the incidenceof atrial fibrillation, persistent atrial fibrillation in patients with left atrialenlargement is more obvious, indicating that atrial remodeling persisted in thepathophysiology of atrial fibrillation, the persistence of atrial fibrillation alsopromote atrial structural changesMultiple regression analysis showed that patients with atrial fibrillationthe aldosterone, CRP, BNP levels and left atrial diameter was a linearrelationship, suggesting that aldosterone, CRP and BNP induced atrialstructural remodeling and promote the occurrence and development of atrialfibrillation.
