Objective:Chronic salpingitis constitutes the major reason of tubal factor subfertility and ectopic pregnancy, which severely affects woman reproduction health. Toll-like receptors(TLRs) is one of important innate immune family receptors. TLR2and TLR4are expressed moderately on the oviduct to emiante pathogeny, while expressed excessively or deficiently, pathologic change of the oviduct will occur. TLR2and TLR4trigger the immune response by identifying the pathogen-associated molecular of patterns (PAMPs) and are involved in the development of chronic salpingitis. TLR2and TLR4can recognize the PAMPs of chlamydia trachomatis which is an important pathogen of chronic salpingitis. In this study, aimed to compare the differential expression of TLR2and TLR4in tissues of the human oviduct between normal, mild and severe chronic salpingitis. At the same time, we investigated the differentiation of the level of chlamydia trachomatis antibody IgG (CT-IgG) in the serum of peripheral blood, which was an important indicator of a previous C. trachomatis infection. We try to find the relationship between expression of TLR2and TLR4and chronic salpingitis, and expect to explore the possible mechanism of TLR2and TLR4in the development of the pathologic change of the oviduct, and eventually provide guidance for the prevention and treatment of chronic salpingitis.Methods:39cases with salpingectomy for severe salpingitis or benign diseases of uterus or/and adjuncts were enrolled. According to the morphology of the oviduct and adhesion with around tissues at the time of surgery, the cases were devided into3groups. Group with normal oviduct (13cases):oviducts was normal, without pathology such as distension, stiffness and adhesion; Grourp with mild salpingitis (13cases):oviduct with distension(<1.5cm), periadnexa fibroid adhesion;oviduct with severe salpingitis (13cases):oviduct with obvious distension(≥1.5cm)、stiffness、extensive dense adhesions with ovary and other tissues in the pelvic and hydrosalpinx.The differential expression of TLR2and TLR4in the oviduct were analyzed by real-time RT-PCR and immunocytochemistry, and the level of Chlamydia trachomatis antibody IgG (CT-IgG) in the serum of peripheral blood was analyzed by ELISA.Results:1、Compared to that in group with normal oviduct, the expression of mRNA TLR2in group with mild salpingitis decreased without significant difference (P>0.05), but that in group with severe salpingitis decreased with significant difference(P<0.05); and there was significant difference between the two groups with salpingitis (P<0.05)2.. The expression of mRNA TLR4in the two groups with salpingitis were both significantly lower than that in group with normal oviduct(P<0.05), but that in the group with severe salpingitis was lower than that in the group with mild salpingitis without significant difference (P>0.05)3、TLR2and TLR4were expressed on the glandular epithelia cells but not on stromal cells of fallopian tube by immunocytochemistry.4、Compared to that in group with normal oviduct, the expression of TLR2in group with mild salpingitis decreased without significant difference (P>0.05),but that in groups with severe salpingitis decreased with significant difference (P<0.05); and there was also significant difference between the two groups with salpingitis (P<0.05).5、The expression of TLR4in group with severe salpingitis was significantly lower than that in group with normal oviduct (P<0.05), and lower than that in group with mild without significant difference(P>0.05), and that in group with mild was also lower than that in group with normal oviduct without significant difference (P>0.05)6、There was no significant difference of the level of CT-IgG in the serum of peripheral blood among the three groups (P>0.05) Conclusion:1、TLR2and TLR4are expressed on the glandular epithelia cells of fallopian tube. TLR2and TLR4are involved in the immune response of the fallopian tube to infection.2、The expression of TLR2and TLR4in oviduct with chronic inflammation significantly decrease, and the degree of decrease become greater with severer sapingitis. The lowered expression of TLR2and TLR4in oviduct might be involved in the pathogenesis and progresson of chronici salpingitis...
