Objective:End stage kidney disease patients tend to merge the fluid and sodium stay, metabolic acidosis, anemia, high blood pressure and other complications, when the organism developed a metabolic acidosis, H+can slowly through the blood brain barrier and make it a metabolic acid toxicosis c status and thus the game from the effects of anesthesia, owing to merge these complications, end stage kidney disease patients are often high blood circulation thus effect his quality of spinal anesthesia, Our research was settled to compare the onset of algesthesia and motor blockade, duration of algesthesia and motor blockade, between end stage renal disease patients and patients with normal renal function.Methods:The CKD group were enrolled who were prepare to operate into the peritoneal dialysis tube between January2010to October. The choice of the control group was perineum or lower limbs surgery patients. Fifteen patients with normal renal function and fifteen patients with ESRD received spinal anesthesia with3mL of0.5%hyperbaric ropivacaine. onset and duration of sensory block, onset and duration of motor block. was observed, cerebrospinal fluid samples was drawn before ropivacaine was given for assay of PH, PCO2、HCO3-. inclusion criteria:no abnormal liver function,age from18to65years, body mass index<30kg/m2,no invigorative system disease and mental disease,no drug and drinking addictive,no long-term use of calm or opoid medicine medical history.the spinal anesthesia punction point is L2-3interspace, spinal medicine is0.5%hyperbaric ropivacaine3ml. we use SPSS13.0to make statistical analysis,Statistical significance was assumed when P<0.05.Results:onset and duration of sensory block and onset and duration of motor block in the ESRD patients are shorter than that in the normal renal function patients(P<0.05). PH of cerebrospinal fluid in the ESRD patients are lower than that in the normal renal function patients(P<0.01), HCO3-of cerebrospinal fluid in the ESRD patients are lower than that in the normal renal function patients(P<0.05), while the PCO2were similar between the groups.(P>0.05)Conclusion:onset and duration of sensory block and onset and duration of motor block in the ESRD patients are shorter than that in the normal renal function patients who undergoing spinal anesthesa with3mL of0.5%hyperbaric ropivacaine.
