Objectives:this study sought to use320row CT portal vein3d imaging system, thispaper discusses the technology of portal vein hypertension (PHT) preoperativediagnosis, evaluating the operation scheme and the formulation of the clinicalapplication effect.Methods:43cases were diagnosed as portal hypertension patients go320rowvolumetric CT examination, and in a normal adult43cases as control group.Scan lower esophagus here to a range of bilateral iliac crest level, usingnon-ionic contrast iodine about80ml. The acquisition of the portal veinoriginal data1.0mm thin layer of reconstruction, the reconstruction ofinterval0.5mm, reconstruction methods including volume reproducetechnology (VR), maximum strength projection (MIP) and planereconstruction (MPR), respectively show and its major branches of the portalvein and the collateral vessels, and by two experienced doctors were measuredthe portal vein, spleen vein, superior mesenteric vein and left gastric vein pipediameter. Measurement data using independent t test sample the statisticalanalysis.Results:the normal group and5class above the portal vein branch show rate was100%. The control group cirrhosis portal hypertension in those caused by38 patients are different degree varicose veins, the portal vein and its mainbranches, spleen vein increased obviously coarse, can display3-the level4branches, part of the patients with portal vein and/or branch with thrombosisor cancer in bolt form; Pancreatic cancer are caused by the portal hypertensionlimitations of the portal hypertension. Portal vein spongiform degenerationpatients normal portal system structure disappear in the portal vein ZouXingdirection, the author dilated collateral vein formation.Cases of patients with portal hypertension, the portal backbone, spleenvein, superior mesenteric vein and left gastric vein direct were15.5+/-2.05,12.8+/-1.23,11.3+/-1.46,5.7+/-1.05, significantly greater than the controlgroup and statistical significance; Among them are spleen-renalvein/stomach-renal vein bypass patients portal backbone diameter is about12.6+/-1.98, without the shunt diameter is about14.9+/-2.11, there aresignificant differences, explain the existence of the shunt in the diameter haveinfluence.When the portal hypertension via more collateral circulation, which leftgastric varicose veins and stomach esophagus-the proportion of varicose veinsmost,74.41%,81.39%respectively.26cases of liver cirrhosis portalhypertension patients were done by gastroscope inspection, including21caseswere confirmed in stomach esophagus-the varicose veins, prove320rowvolumetric CT and upper stomach esophagus-in diagnosis of varicose veins ina high degree of coherence.Conclusion:320row volumetric CT portal venous blood vessels3d imaging system,can clear, accurate, comprehensive display of the portal vein and its collateralcirculation change, and to evaluate the degree of disease provide reliable clinical information, for understanding of the system before portal veinanatomic structure and evaluation of the feasibility and surgery operationmethod provides important reference value.
