Background: Recent studies have found that adipose tissue as endocrineorgan release a large number of biologically active substances, such as visfatin,adipocyte fatty acid binding protein (A-FABP), these substances has beenshown to have the role of inflammatory effect, insulin-mimicking effects,regulate glucose and lipid metabolism disorders, and to involve in theoccurrence and development of atherosclerosis. So recently, majority ofresearchers focus on the effect and mechanism of adipose tissue in the coronaryheart disease, such as visfatin, A-FABP.Objective: The aim of this study was to investigate by comparingdifferent degree of coronary atherosclerosis in patients with serum levels ofvisfatin, A-FABP, IL-6, and explore the relationship among visfatin, A-FABP,IL-6and the severity of coronary artery pathological changes in patients withthe severity of coronary stenosis, assess the clinical value of serum visfatin,A-FABP in the prediction of coronary atherosclerosis extent, quest a new targetfor prevention and treatment of coronary atherosclerosis.Methods: According to inclusion and exclusion criteria, select88cases ofpatients with suspected coronary artery disease in cardiovascular treatmentcenter, the first Bethune hospital of Jilin university from August,2011toDecember,2011.66cases of patients with coronary heart disease confirmed bycoronary angiography in clinical,22cases of normal coronary arteries confir-dby coronary angiography, the study was designed as a prospective case controlstudy. All selected candidates were measured height, weight, blood pressure,fasting blood glucose, lipids, liver function, kidney function, etc. Serum levels of visfatin, A-FABP, IL-6were determined by double antibody sandwichenzyme linked immunosorbent assay (ELISA). Experimental data wereanalyzed by SPSS17.0statistical software. The enumeration data werecompared with chi-square test; measurement data of the Normal distributionwere expressed as mean±standard deviation, t text for the pairwise meancomparison; and mean comparison among groups was apply one-way analysisof variance. Measurement data of the non-normal distribution or variance ofnon-homogeneity compared with Mann-Whitney rank sum test; bivariate linearcorrelation analysis for evaluating the correlation between two factors; Applymultiple linear stepwise regression analysis for assessing the relationship ofrisk factors. Using a Receiver-Operating Characteristic (ROC) curve analysisthe clinical value of serum levels of visfatin, A-FABP and IL-6in patients withcoronary heart disease. Two-side values of P<0.05were considered statisticallysignificant.Result:1. Physical characteristics: Compared with normal control group,smoking, hypertension, diabetes, hyperlipidemia, systolic blood pressure,diastolic blood pressure, body mass index were significantly increased in theCoronary Heart Disease (CHD) group (P<0.05).2. General laboratoryexaminations: Triglyceride (TG), Total cholesterol (TC), high densitylipoprotein-cholesterol (HDL-C), Low density lipoprotein-cholesterol (LDL-C),Fasting blood glucose (FBG), serum uric acid(SUA) in CHD group increasedsignificantly compared with normal control group (P<0.05).3. Serum levels ofvisfatin,A-FABP,IL-6: As aggravation of coronary atherosclerosis extent innormal control group, mild atherosclerosis group, moderate atherosclerosisgroup, severe atherosclerosis group, the serum levels of visfatin, A-FABP, IL-6gradually increased; and comparison among four groups were significantdifferences (P<0.05).4. Linear correlation analysis: Bivariate linear correlationanalysis suggest, there was a positive correlation between Gensini score and Serum levels of visfatin, A-FABP, IL-6(P<0.05); and there was a positivecorrelation between IL-6and Serum levels of visfatin, A-FABP (P<0.05).5.Multiple linear regression analysis: Visfatin, A-FABP, IL-6is the risk factorsfor coronary heart disease, order on the role of coronary heart disease severityby visfatin、IL-6、A-FABP.6. ROC analysis: The area under ROC curves ofSerum visfatin, A-FABP, IL-6were0.90、0.83、0.85respectively for predictingseverity of coronary artery pathological changes, difference was statisticallysignificant compared with reference line (P<0.001). The accuracy rate was at amoderate level. When serum visfatin, A-FABP, IL-6take the demarcation point54.98ng/ml,19.92ng/ml,98.57pg/ml respectivly, the corresponding valueswere the best demarcation point.Conclusion:1.There was a positive correlation between severity ofcoronary atherosclerosis and serum levels of visfatin, A-FABP, IL-6, therewere both participate in the formation and development of coronaryatherosclerosis, there were the non-traditional risk factors of coronary heartdisease; and compared with A-FABP and IL-6, the role of visfatin is the largeston assess severity of the coronar atherosclerosis.2. There was a positivecorrelation between IL-6and Serum levels of visfatin, A-FABP, thedevelopment of the severity of coronary atherosclerosis may have a synergisticeffect.3. By ROC analysis, the best demarcation point of serum visfatin,A-FABP, IL-6, expected to become a reference index for predicting theseverity of coronary atherosclerosis.
