The Risk Factors For Postoperative Complications Of Obstructive Jaundice In Elderly Patients

Objective：Multivariate analysis of the risk factors of the postoperativecomplications in elderly patients with obstructive jaundice,in order to provideguidance for preoperative preparation and prevention.Methods：A retrospective analysis of the surgical treated of102elderlypatients with obstructive jaundice from June2005to December2011, in theHepatobiliary Surgery of China-Japan Union Hospital.Results：(1) In this group of102patients, there were39cases of one ormore postoperative complications (38.2%), wound dehiscence or fatliquefaction nine cases (8.8%), infectious complications occurred in17cases(16.7%), bleeding complications occurred in2cases (2.0%), anastomoticfistula in six cases (5.9%), delayed gastric emptying in six cases (5.9%),postoperative ventricular dysfunction in1case (1.0%) MODS four cases (3deaths)(3.9%).(2) Multiple regression analysis showed: an independent riskfactor for postoperative complications preoperative jaundice, preoperative ALB,organ systems, combined diseases; an independent risk factor for infectiouscomplications is the major organ systems of comorbidities; hemorrhagecomplicated preoperative ALB, an independent risk factor for disease. Delayedgastric emptying, an independent risk factor for complications of preoperativeALB levels and direct bilirubin levels; preoperative total bilirubin level ofindependent risk factors for anastomotic leakage; an independent risk factor formultiple organ dysfunction (MODS) in preoperative the jaundice time, whiteblood cell (WBC) level, serum creatinine (Cr) level.Conclusions：The days of jaundice last, preoperative ALB serum level,an important organ system complications, preoperative total bilirubin and direct bilirubin levels,serum Cr level are independent risk factor for elderlyobstructive jaundice postoperative complications.
