The Expression Of HIF-1a、VEGF、Endostatin In And The Survival And Prognosis Of HCC Patients With Preoperative TACE Treatment After Hepatectomy

Objective This study by a retrospective analysis of preoperative hepatic artery chemoembolization (TACE) group and TACE surgery group, the clinical data of patients with hepatocellular carcinoma and detection of HIF-1a, VEGF, of Endostatin expression and prognosis to explore preoperative TACE on liver resection of patients with efficacy and prognosis of cancer.Methods (1)We retrospectively analyzed 51 patients with cases admitted to our hospital because of hepatocellular carcinoma (HCC), hepatobiliary surgical liver resection and confirmed by pathology and clinical data integrity. Grouped according to whether the patient before surgery by TACE. Divided into preoperative TACE group (experimental group n= 33) and did not undergo preoperative TACE group (control group n = 18). (2) The TACE group by SABC immunohistochemical method for detection of preoperative TACE group and preoperative, not HIF-la, VEGF, factor of Endostatin expression with clinical data and postoperative follow-up results. (3)application the SPSS 13.0 system juice software, clinical data between the two groups compared by x2test, P <0.05 was considered significant difference. Survival rate between the groups using the Kaplan-meier analysis, Log Rank test. Results (1) The two groups in gender, age, number of lesions, whether the tumor invaded the liver capsule, with or without HBV infection were undifferentiated. Preoperative TACE group, tumor size 5cm up to 84.4%(28/33) significantly more than non-TACE group,44.4%(8/18) between the two groups was significant difference (x2= 9.158, P= 0.002). (2) Two groups of patients with HIF-la and VEGF in factor of Endostatin expression:HIF-la staining o;the nucleus and cytoplasm, TACE group of patients with preoperative positive rate was 69.7%(23/33), non-TACE group, the positive rate 72.2%(13/18); VEGF, of Endostatin positive expression in the cytoplasm, the preoperative TACE group, the positive rate was 63.6%(21/33) and 57.8%(19/33), non-TACE group,61.1%(11/18) and 55.6%(10/18). (3) Comparison of the recurrence and prognosis of two groups of patients:after 1 year recurrence rates were 36.4%(12/33) and 33.3%(6/18), there was no significant difference (%2= 0.077, P= 0.782). Two sets of overall survival rate was no significant difference (x2= 1.334, P= 0.248). Conclusion:(1) Two sets of clinical data, pathological differentiation x2 test there was no significant difference, indicating that preoperative TACE does not have a negative impact on the pathologic differentiation of HCC patients. (2) The two groups were compared its of HIF-1a and VEGF in of Endostatin expression and found that expression was no difference between the two groups. Preoperative intervention group of patients with tumor aggressiveness, degree of cell proliferation and invasion of the transfer can achieve the same level with direct surgery in patients with HCC, preoperative TACE treatment is not in HCC invasion, proliferation, transfer adverse effects. (3) The two groups using the Kaplan-meier analysis of their survival there was no significant difference. Therefore, not a surgical resection of liver cancer patients, the preoperative TACE treatment is an effective treatment method.
