Effect Of Follicular Fluid IGF-Ⅰ And Steroid Hormones Levels On Oocyte Maturation With Polycystic Ovarian Syndrome

Objective To observe the difference of follicular fluid IGF-Ⅰ, steroid hormones levels, oocyte maturation rate, fertilization rate and good quality embryo rate between PCOS group and control group. To investigate the relationship between follicular fluid IGF-Ⅰ, steroid hormones levels and follicle diameter, oocyte maturation from patients with polycystic ovarian syndrome (PCOS) during intracytoplasmic sperm injection (ICSI) cycles.Methods Follicular fluid (FF) were obtained from41women with PCOS and37women of contrast group on oocyte pick-up (OPU) day during ICSI cycles.. All the follicles were grouped of≤14mm and>14mm according to the diameter size.3-4follicles were obtained from each diameter group. Estradiol (E2) and testosterone (T) concentrations in each follicular fluid were assessed by electro-chemiluminesence immunoassay (ECLIA), IGF-Ⅰ levels were measured by enzyme-linked immunosorbent assay (ELISA).Results1. In follicles with diameter of≤14mm, oocyte maturation rate and fertilization rate of patients with PCOS were higher than control group (P<0.05). And with the diameter of>14mm, there were no difference in oocyte maturation rate, fertilization rate and good quality embryo rate between PCOS group and control group. FF IGF-Ⅰ, E2and T concentrations in PCOS patients were significantly higher than control group in bothy diameter groups (P<0.05).2. In PCOS group, FF IGF-Ⅰ levels were positively correlated with T and E2(P<0.05).3. The oocyte maturation rate (85.7%) was the highest in PCOS group(P<0.05) with FF E2/T>140, in control group the rate was the highest (73.4%) with FF E2/T between75and110.4. With the diameter of≤14mm FF IGF-I levels in MⅡ period and normal fertilization were higher than relative M I+GV periods and abnormal fertilization., and in PCOS group FF IGF-I levels in embryo of A period were higher than embryo of B+C+D periods (P<0.05).Conclusion The increased FF IGF-I, E2and T levels in PCOS were not correlated with follicle size on OPU day. Proper ratio of E2and T in FF may affect oocyte maturation. Oocyte maturation and development potentiality were effected by IGF-Ⅰ And high levels of FF IGF-Ⅰ in PCOS would mainly affect oocyte maturation and fertilization when follicle size of≤14mm.
