Objective: The observation of hypoxia-inducible factor (HIF-1a) expression inesophageal squamous cell carcinoma tissue and serum and its relationship with theefficacy of chemotherapy. The HIF-1a may predict experimental data and the basis foresophageal squamous cell sensitivity to chemotherapy.Methods: We had collected asymptomatic population gastroscopy census innewly diagnosed30patients with esophageal squamous cell carcinomas in Luoyangarea of Henan province, We used immunohistochemical SP method with the purposeof detect the expression of HIF-1a protein in squamous cell carcinoma; All of patientswith cisplatin/paclitaxel based chemotherapy, The taxol is135mg/m~2,3h ivdrip d1;The cisplatin is25mg/m~2, ivdrip d2~d4. The one cycle is21days. Each patient hadcompleted at least two cycles of chemotherapy. The collection serum of patientsbefore and after chemotherapy, Using enzyme linked immunosorbent assay (ELISA)method to detect HIF-1a serum concentration changes before and after chemotherapy.Then observation CT of patients before and after chemotherapy, evaluation oftreatment efficacy. Use SPSS software to analyse the expression of HIF-1a betweentissue, serum and the relation the effect of chemotherapy.Results:1. The HIF-1a is expression in esophageal squamous cell carcinoma and normalesophageal mucosa: The rate of organization expression of HIF-1a positive expressionwas63.3%(19/30) in esophageal cancer, Clinical late incidence in cancer tissuepositive expression and lymph node metastasis were significantly higher than thenegative group (89.5%VS.45.5%,89.5%VS.36.4%), The two groups werestatistically significant (P<0.05). The poorly differentiated incidence is no significantdifference between the two groups (47.4%VS.27.3,P>0.05).2. The HIF-1a is expression in the serum of esophageal squamous cell carcinoma: The HIF-1a of serum concentration which positive expression is:99.26±27.32pg/ml, The HIF-1a of serum concentration which negative expression is:78.28±26.91pg/ml. The expression of positive group is higher than negative group, therewere statistically significant (P<0.05).3. The relationship of chemotherapy efficacy is between HIF-1a of expression intissue and serum concentration in esophageal squamous cell carcinoma: Afterchemotherapy, in the30cases of the patients had arrived CR2cases, PR10cases, NC13cases and PD5cases. The overall effective rate (CR+PR) was40%. The effectiverate was15.8%in the HIF-1a positive expression, but negative expression group was81.8%, the positive expression group is lower than negative group. The two groupswere statistically significant (P<0.05).4. The relationship of chemotherapy efficacy and HIF-1a serum concentration:The serum concentration of availability treatment (CR+PR) was75.72±26.23pg/mlbefore chemotherapy; the serum concentration of unavailability treatment (NC+PD)was98.13±21.06pg/ml before chemotherapy. The effective group of averageconcentration is lower than the inactive group, Two groups were statisticallysignificant (P<0.05). After chemotherapy, the serum concentration of25cases patientswere decline, the12cases of them had chemotherapy effective. The effective rate ofserum concentration is48%(12/25). The serum concentration of5cases patients wasrising after chemotherapy, The chemotherapy effective rate is0%(0/5).Conclusion:1. The protein expression of HIF-1α is mostly positive in esophagealsquamous cell carcinoma. Normal esophageal is negative. In positive expression, theclinical stage is later and lymph node metastasis is significantly higher. It suggests thatHIF-1a may play an important role in the development of esophageal cancer and aboutits invasion of metastatic.2. There is consistency by immunohistochemistry detection HIF-1a proteinexpression in tissue with by ELISA detection HIF-1a serum concentration. It suggeststhat the later can be used as one effective way to response the level of HIF-1a inesophageal cancer3. In esophageal squamous cell carcinoma, the HIF-1α protein expression ishigher, and the serum concentration is higher, but the chemotherapy effect is muchpoorer. It suggests that HIF-1a can be used as one of the indicatort to predict thechemotherapy sensitivity in esophageal squamous cell carcinoma.
